In this assignment you are asked to apply and examine various models of accident / incident causation and risk assessment / mitigation (referencing credible sources) to create a risk mitigation report of a professional (workplace) standard of quality, that responds to the scenario provided below. Referencing credible sources involves examining and evaluating ideas from a variety of credible sources. Specifically, in this assignment you are asked to respond to the scenario (provided below) by

1. Choosing an accident / incident causation model to identify the likely issue for concern.

2. Presenting a list of readily identified hazards.

3. Choosing five (5) of the above hazards in the context of the scenario and provide a brief description (approximately 80-100 words) for each. When describing these hazards, pre-existing controls that are already in place can be included. It is likely that you will need to make various assumptions at this point.

4. Designing a risk management system which could be used to manage one (1) of the hazards described above.

In practical terms, this means you must relate all five of the steps or elements of the risk management process to this issue (approx. 1,000 words).What you need to do Risk Mitigation Report You are asked to write a report that is approximately 2,000 words long (maximum). It must be formatted correctly and contain:

• Letter of Transmittal (does not contribute to word count)

• Title Page (does not contribute to word count)

• Table of Contents (does not contribute to word count)

• Executive Summary (does not contribute to word count)

• Introduction (contributes to word count - approx. 200 words)

• Body of the Report (contributes to word count - approx. 1,500 words)

• Recommendation (contributes to word count - approx. 300 words)

Referencing: ? use in-text references to acknowledge sources in the body of the report. ? include a List of References (listing full bibliographic details) on a separate page at the end of the report Points to note:

  •  This assignment is not testing your knowledge of heavy transport depots or fencing. Rather, the focus is on testing your ability to apply risk management to a particular workplace problem. The focus should therefore be on Module 3 on causes of incidents/accidents and Module 4 on how to manage your identified hazard.
  •  Assume your assignment will be read and discussed by the company owner. Keep this in mind in arranging your arguments.
  •  A report format is the prescribed approach. Use can be made of headings and sub-headings to provided structure to your work. You decide how this may be done in an effective way. ? Spelling, punctuation, and grammatical errors are similarly unacceptable in assignment work. Marks will be deducted for the above.
  •  Finally, there is no single right answer. Rather, you are assessed on your ability to construct useful argument, link to suitable reference materials and support your position.
