1A: Counselling Practice (1-3)

  1. Submit one (1) case note chosen from either session 2 or 3.
  2. Submit a copy of the Intake Form
  3. Submit the Case Conceptualisation form outlining how you will proceed with your client. This needs to be linked to the goal and the counselling modality you will be using for future sessions.
  4. Complete a 750-word evaluation report on the first three sessions to address the following:
    • The presenting issue and goal that emerged for the client.
    • The long-term goal for counselling and an indication of what the client wants to be different in the future.
    • How you will use the case conceptualisation form to help you plan the next stages.

1B) Working Stage

  • Submit one (1) case note from the middle/working stage of counselling practice for assessment (session 4, 5, 6). The case note is to show application of any relevant feedback provided for assessment 1A. The focus is on developing reporting and case note writing skill and therefore the note contains counsellor hypothesis and clinical thinking in the Assessment section and the recording of therapy interventions in the Plan. It is to be written in a style that would enable another counsellor to continue working with this client if needs be. (500 words equivalent)
  • Complete a 1000-word evaluation report on the middle/working stage sessions.
  • The report should be supported by theory and research showing a depth of understanding of the client problem. It therefore requires referencing of material that is not your own original thought.
  • Reflecting on the case note submitted and focusing significantly on the Assessment & Plan sections of the SOAP note address the following:
    • Drawing on what you understand of counselling theory, why did you choose this counselling theory/modality for your client and their issue? How does it align with their goal?
    • Outline how you have attempted to apply the interventions linked to this modality; this includes homework tasks?
    • What change/shift in your client did you notice over this middle/working stage of counselling? What caused this shift to come about?
  • Outline what you are considering next for the client? This will include a revised plan and goal for the final stage of counselling.

1C - Termination Stage

Incorporating information taken from your case notes from this final/termination stage of counselling practice for assessment (session 7, 8, 9) -

Complete a 1500-word outcome report and detail the following in your report:

  • Evaluate any changes in the client’s presenting issue with consideration of the Intake information.
  • Your perceived understanding of the deeper issue perhaps not stated by the client. This is the application of counsellor hypothesis and clinical thinking, so is to be supported by researched and referenced theory that relates to the client and their clinical/social issue.
  • Outline the redefined goal and how you adapted and/or integrated theories and approaches to support this new goal.
  • Provide a critical reflection of your counselling process including a summary of the modality/interventions/tools used. Evaluate the effectiveness and limitations of your counselling skills and practice and the effect this had on the client change process. Include areas for your personal growth and development, and an action plan of how you will achieve this on field placement.
  • Outline how the therapeutic work in session is to be applied post-counselling into the broader context of the client’s world.
  • Provide an overview of the key points learnt from peer/subject coordinator supervision. Include how you will apply and synthesise these points in the future.
  • Provide one referral source that may be appropriate for your client and discuss your reasons.

Report
