Identifying two person-centred support that promotes self-management

Self-management becomes crucial for chronic patients to regain the ability to control emotions, behaviours and regular activities. It has been observed that chronic patients often require community support services to ensure person-centred care support in their daily lives. According to the case study, Joseph was admitted to the hospital due to an acute respiratory infection along with his critical medical history of COPD since 2015. He has been provided with essential medications and treatment on immediate note. The care support worker who took care of Joseph informed me that his mobility has decreased along with the rising problems of breathlessness. Joseph lives alone at his home and became adamant to go back to home by avoiding medical follow-ups. Eventually, neighbours of Joseph are older adults who suffer from severe health issues and it is not possible for them to take care of Joseph all the time. In this context, two person-centred needs have been identified for Joseph from the perspective of the registered nurse. The first one is a respiratory care nurse from the local community centres to provide guidance, support and regular medication to Joseph (Benzo & McEvoy, 2019, P. 1069). The second support that can help Joseph in self-management is support groups. These two identified person-centred support can help Joseph as well as similar chronic patients in self-management at home.

Discussing the rationale for selecting one of the specific support needs

The community support of a respiratory care nurse has been identified here as the patient is currently suffering from acute respiratory infection along with a severe medical history of COPD. Joseph, the patient became adamant about going back home without attending the medical follow-ups in the hospitals. However, he lives alone and does not have anyone who can take care of him when the health situation is critical. As per the studies of Hoogendoorn et al. (2021, P. 104005), respiratory nurses work directly to provide support and educate patients and their families regarding respiratory health. For chronic patients, respiratory nurses manage the day-to-day health conditions of patients. Health assessment, ongoing support, care planning, proper medications, and supporting patients with inhalers and other techniques are considered major responsibilities of respiratory nurses. Joseph needs exactly the same continuous support at home to improve self-management and thus respiratory care nurse has been recognised as an essential person-centered support.

Resources that can support the person in the selected scenario

Some resources are required to enable all support for the patient in the given scenario. It includes both organisational and multidisciplinary supports that can help the respiratory care nurse in taking care when required. In this regard, the support of the hospital and local community centre can be very effective in supporting the patient. Accordingly, medical and financial resources are also required to ensure medications and other necessary medical equipment are on time when needed.

Identified social barrier

As narrated by Marks, Edwards & Jerge (2021, P. 167), respiratory care nurses are highly trained as they require specialised skills and knowledge. Therefore, hiring a respiratory care nurse on a daily basis can be highly expensive. Usually, community care centres in Australia charge higher amounts to provide respiratory care nurses compared to RNs (general nurses). Contrarily, Joseph is retired now and does not have any means of earning in the current situation. He entirely depends on his savings and the financial support provided by his son. This financial constraint can be a social barrier in terms of availing essential care support.
