Aim
In this assessment item you will have the opportunity to demonstrate the progression of your critical thinking skills by planning nursing care in partnership with a person who has a chronic condition. Nursing care planning enables nurses and student nurses to demonstrate their clinical decision-making capabilities and supports safe evidence-based practice.
This assessment item will assess:
Learning Outcome 1: Demonstrate theoretical knowledge, practical skills, and interprofessional and cultural capabilities that underpin safe and competent nursing care for people living with chronic conditions in a range of nursing practice contexts; &
Learning Outcome 2: Create an evidence-based plan of nursing care for the person with a chronic condition, that is focused on optimising self-management and health promotion in primary health care.
Task Description
Within Modules 2 and 3 you have explored chronic condition management and nursing care plans.
Examine ONE of the following case studies, and with reference to appropriate scholarly literature, write a 1500 word care plan in which you:
Identify TWO (2) potential or actual chronic condition concern/problem for the patient described in the case study and justify your selection (this may include mortality & morbidity statistics, specific burden of disease for the case study or other justified reasons);
Identify patient data relevant to the potential or actual chronic condition concern and specify any additional assessment techniques or tools you would use to collect patient data;
Identify and prioritise TWO (2) goals for care of the patient in planning the nursing care (ensure these are SMART goals);
Describe and justify TWO (2) evidence-based nursing interventions you would implement for EACH of the identified goals; and
Predict expected outcomes and describe how these would be evaluated for each nursing intervention.
Case Study 1:
Identification: Ivy Brown is a 48-year-old novigender woman who has had a high BMI since early teenage years. She loves music and attending live concerts. Ivy hates exercise and hates feeling sweaty. Ivy works as a teachers aide at the local primary school, and she volunteers in the local band as a flutist. Ivy has self-diagnosed irritable bowel syndrome (IBS) and avoids going out if she doesnt know where the toilet is at the venue. Ivy has negative feelings about what she has achieved so far in her life.
Situation: You have come to know Ivy as she brings her mother, Muriel, into the GP clinic where you work as a practice RN. She made an appointment to see you a week ago about her general health and long term back pain.
Observations: Her vital signs are: HR 125bpm regular, SpO2 97% on RA, Temp 36.5 C, RR 24 bpm, BP 148/94mmHg. She describes back pain that wakes her in the night (rated 7/10) and daily pain (rated around 3/10). You note that Ivys spine is in alignment with a small amount of tenderness over lumbar (L4) vertebrae. Ivy describes feeling bloated, but her abdominal assessment is normal.
Background: Ivy tells you that she is really tired of the back pain and that she thinks she wants to make changes in her life before she gets too old. She treats her back pain with Panadol and Nurofen and says that she has a script for Endone that she knows is stronger pain relief but that she feels scared about taking it in case she gets addicted. This is the first time she has been to see anyone about her health in a year. Ivy doesnt believe in checking for breast or cervical cancer as, if it is your time to die, then that is the way it goes.
These are Ivys medications:
Paracetamol 1g 4/24 (no more than 4g /day)
Ibuprofen 400mg 4-6/24
Oxycodone 5mg PRN (not ever taken yet)
Glucosamine 500mg TDS
Metamucil 1 Tbsp mane
Mylanta 10-20 mls PRN
Responsibility: You have decided that Ivy could benefit from nursing interventions about chronic health concerns.
Case Study 2:
Identification: James Clark is a 68-year-old cisgender man who is travelling around Australia with his wife Geraldine on the trip of a lifetime with a 4WD and caravan. They left Queensland 5 months ago. When he unhooked the caravan last night, he noticed tingling in his left arm and he felt short of breath. He sat quietly for 30 mins and the feeling passed. James really doesnt like to make a fuss. He woke up during the night to go to the toilet and felt dizzy. Geraldine has insisted he get checked at the hospital before they start the drive across the Nullarbor Plain.
Situation: You are working in an Adelaide ED and James is a patient in the sub-acute area.
Observations: His vital signs are BP 135/90mmHg, HR 95 BPM regular, RR 22, SaO2 97 % on RA. Temp 36.9 degrees. His ECG has no acute changes (No ST elevation or ST depression). Air entry is equal in both lung bases and no crackles or wheeze evident. He has a superficial wound on his left lower leg (5 cm x 5 cm) that he states was a scratch from a piece of firewood that I mishandled but its been there for a month now. There is no elevation in his troponin blood level and his GCS 15/15 and CT head is normal.
Background: James has had asthma since childhood with periods of exacerbation of asthma related to seasonal pollen in the air and infrequent chest infections. James has brought his medication in to the clinic and shows you a list of the medication he takes (he states he doesnt really know what all his medications do).
These are James medications:
Metoprolol 25mg daily
Hydrochlorothiazide 12.5mg daily
Zinc 10mg daily
Caltrate 600mg daily
Salbutamol X2 puffs PRN
Symbicort X2 puffs BD
Responsibility: You have been allocated the care of James and decide that James could benefit from nursing interventions about his chronic health concerns.
