Your submission should be referenced using the APA 6th Edition. Guidance on how to do this is available on the library gatewayYou should format your work using size 12 Cambria or Arial font. Your paragraphs should be formatted in 1.5 line spacing. Please include a front page detailing your name, module title and module leader and your word count.

Task details and instructions: You are required to produce a case study that demonstrates your critical appraisal skills in identifying and reviewing best evidence in the management of your chosen long-term condition in the local, national and global context. Within your case study you will be able to demonstrate how you will make informed judgements based on this evidence and how this will change or improve future practice. Provide an introduction to a long-term condition that is relevant to your own area of practice that requires management, illustrating why it is relevant/important.

Identify, select and synthesise the information required to effectively assess, diagnosis, investigate and manage the patient with a long-term condition.

Justify effective and efficient patient management strategies utilising evidence-based practice, apply and justify appropriate methods for audit and clinical evaluation of services for people with long term condition

Critically reflect upon current theories and concepts underpinning professional practice and their clinical application in generic and specialist management of a variety of long-term conditions.

Critically discuss the structure, policies and wider context of chronic disease management and evaluate their impact on patients with these conditions.

Ensure organisation and presentation is evident in the work and that all guidance has been followed. Demonstrate a satisfactory level of skill in relation to sentence structure and grammar.

Weighting within module: This assessment is worth 100% of the overall module mark.

Word count/ duration (if applicable) The assignment should be 5000 words in length. This word limit relates to the word count given by the word count tool in Microsoft Word and should be included at the end of the document before the reference list. Work that is >10% over the word limit will be penalised by 10% of the available marks, i.e. you would lose 10 marks on this assessment task. Note that all text, apart from the list of references at the end of a document and text in appendices, is included in the word count.

Follow guidance above on what is to be included in the appendices. Please note that appendices should not be used as a mechanism to expand the volume or length of an assessment task.

As a guide you should consider allocating approximately one third of your word count on the identification, selection and synthesise of the information required to effectively assess, diagnosis, investigate and manage the patient with a long-term condition.

All marks and feedback are internally moderated and a sample is seen by the external examiner
