Guidance for this assessment

The aim of this work is to develop your understanding of the science underlying a major disease, your appreciation of how our understanding of a disease is built up from the results of many studies, and your ability to evaluate evidence.

Specific skills objectives are to further develop your competence in:

Your case study report should be sectionalised in a mannersimilar tobelow:

Introduction:In this you should outline the scope of the report, i.e. saying what you are going to discuss and what your main points are. It is important that you dont just go straight in giving details about the disease. In this section you wish to consider the symptoms and make some generic points.

Body:In this you present your ideas. All material should be relevant to the subject of the study. The main points and issues should be explored andevaluatedand conclusions justified. Make sure your written work really addresses the title. Avoid just describing facts about the disease or the content of any papers, without explaining how this relates to what we understand about the disease. There should also be discussion on the differential diagnosis and the evidence that led you to focus on thisparticular disease. We are looking for evidence of understanding of any methods usedand the ideas discovered,so we expect it to be explained in your own words. Excessive reliance on direct quotes from source texts and the use of undefined jargon suggests a lack of understanding. Successive paragraphs should be linked so that there is a definite structure.

Conclusion:This should be asuccessful drawing together of the various points made. The report should not end abruptly and/or simply repeat the introduction.

References: These should be clear and consistently accurate usingthe Harvard systemin the text and in the references section.

Case Study Questions:Each case study is accompanied by a set of questions aimed to guide you through the case study and maintain your focus. These questions must be addressed and incorporated within the appropriate sections of your work.

Approximate marks are given for each question to as a guide as to how much you are expected to write. You will be given an overall mark as a standardised percentage.

COMENSUS reflection: As part of this module, you will be taking part in a COMENSUS(Community Engagement & Service User Support)session and interviewingpatientswith a condition discussed in the module (please note the patients mayhave a different condition to that in your case study). You are to provide approx.250 words personal reflection on your session with the patients.Here you may consider the impact living with a condition can have on the patient, what you personally gained from this session and what you may do differently in the next COMENSUS session.

Case study

A 65-year old man presents to the doctor after noticing that he has developed shortness of breath and chronic cough that is progressively worsening. The cough is particularly bad in the morning with production of excessive sputum. Upon further questioning, he explains that he works in an office, his general health is good, and that he has never suffered from allergies. He drinks alcohol occasionally and has smoked one to two packs of cigarettes per day for the last 40 years. Heart and abdominal examinations appear normal.

Discuss a likely diagnosis for this patient including aetiology, risk factors and epidemiology. (20%)

Describe how this condition can be distinguished from other common respiratory conditions. (20%)

Discuss the pathophysiology underlying the symptoms experienced by this patient. (30%)

Discuss the most common treatment options available for this patient. (20%)
