(Type your full name in the field above) Work Placement Assessment Instructions

Your work placement involves completing 100 hours of work related activities. While these activities will vary depending upon the organisation in which you do your placement, you are required to conduct case assessment, planning, and monitoring activities with at least three clients.

Specifically, you will be required to:

Collect and analyse client information. This will involve completing organisational intake procedures, interpreting test results and reports, directly observing and questioning at least one client (and their family where appropriate), and documenting information in line with organisational requirements.

Check for and respond appropriately to specific client issues. This will involve reviewing available client information for social, cultural, psychological and/or developmental issues, recognising and referring any potentially serious issues in line with organisational requirements, and applying knowledge of human development to check developmental status in order to identify the presence of any developmental issues.

Plan actions to address identified issues. This will involve planning actions that are appropriate for the needs and circumstances of clients in line with the specific approaches, philosophies and regimes of the organisation as well as the clients developmental level and ability. If there are developmental issues present you will be required to identify potential factors responsible for the significant variations from normal development and plan services accordingly.

Implement services. This will involve implementing, monitoring and reviewing case plans for at least three clients in line with all organisational standards, policies, and procedures as well as legal and ethical requirements.

Additionally, during the work placement, you will also be required to demonstrate an understanding of, and compliance with, all relevant organisational policies and procedures and legal and ethical requirements. Specifically, you will be required to:

Comply with all legal and ethical requirements of your role within the organisation. This will involve fully understanding and complying with all relevant organisational policies and procedures related to legal and ethical requirements (e.g., following procedures related to confidentiality, records management, informed consent, duty of care, mandatory reporting, and codes of practice). You will also be required to engage in a discussion with your workplace supervisor in which you demonstrate your understanding of these requirements by answering questions as well as developing appropriate hypothetical strategic responses that the organisation could make for at least three different situations in which legal or ethical requirements were breached.

Comply with WHS policies and procedures, conduct a workplace risk assessment, complete consultative activities, and coordinated workplace procedures for a simulated emergency situation. This will involve complying with all relevant WHS policies and procedures throughout the work placement (e.g., following procedures, using appropriate personal protective equipment, and reporting hazards according to organisational procedures). You will also be provided with an opportunity to conduct a workplace risk assessment, record the results according to organisational procedures, report the outcomes of the assessment to at least two other staff members, and conduct appropriate consultation activities if required. You will also be provided with an opportunity to coordinate workplace procedures for a simulated emergency situation.

Demonstrate respect for diversity. This will involve respecting diversity in relation to both clients and colleagues, utilising suitable communication strategies to facilitate effective communication and respect for diversity, and discussing the effectiveness of organisational diversity practices with your workplace supervisor.

Your workplace supervisor will work with you to facilitate your access to experiences that will allow you to fulfil these requirements.

You are required to complete a log book of placement hours and activities throughout your placement. This can be found at the end of this Work Placement Assessment Book. Please print it out and complete it progressively throughout your placement. Once your work placement is complete, you must have your workplace supervisor sign-off on your log book and then forward your completed log book to your Work Placement Coordinator.

Additionally, you will be required to document and reflect upon your experiences during your placement within this Work Placement Assessment Book. Some of these questions will require you to outline the work you completed with various clients. It is vital to protect the confidentiality of your clients, as such, you should remove any identifying client information from your descriptions.

It is recommended that you complete this Work Placement Assessment Book progressively throughout your placement. Each question relates to a specific workplace experience or task. You will find it easiest to complete this Assessment Book if you complete each question as soon as possible soon after you have engaged in the relevant activity/experience rather than waiting to the end of your placement. As such, you should take the time to read through this Work Placement Assessment Book before you begin your placement in order to familiarise yourself with the requirements of each question.

If, at any point, you are unsure of the requirements for a particular question, please contact the AIPC Work Placement Coordinator. Failure to submit a complete Work Placement Assessment Book will result in you receiving a not yet competent result and you will be required to resubmit or even re-do your work placement.

We hope that you enjoy your placement experience it is a valuable learning opportunity and one that will help you progress into the start of your new career!

Please provided a brief overview/summary of your placement organisation in your own words. Your overview/summary must include:

The name of the organisation and where it is located

The organisations mission/goals

What services they provide

The key approaches/frameworks the organisation uses

(Your response should be between 100-150 words)

Work Placement Logbook

You are required to complete a logbook detailing all of the activities you undertook on your 100 hours of work placement. On the following pages you will find a logbook that needs to be completed. Please print it out and complete it progressively throughout your placement.

You should record all activities undertaken in the workplace during an assigned work placement shift. These activities might include (but are not limited to) induction, reading policies and procedures, shadowing workers, working with clients, identifying clients needs, supporting clients, engaging in case management activities, referring clients to other services, engaging in WHS-related processes, and formal supervision with your supervisor.

For each shift, your supervisor for those shift activities should sign your logbook to confirm that you completed those hours and activities. At the conclusion of your work placement your assigned workplace supervisor is required to sign at the bottom to confirm that this logbook is an accurate reflection of your placement activities. You must then forward this completed logbook to AIPC.

You are only approved to complete a maximum of 8 hours placement per shift, which includes a 30-minute lunchbreak (Note: the lunch break will not count towards placement hours, however must be documented to ensure you have taken sufficient breaks). If you are asked to complete more than 8 hours per shift or shift outside of normal hours (evening, weekend and/or sleepover shifts) you are required to contact your work placement coordinator to obtain approval, as you will not be covered by the Colleges insurance.

Note: You are not to include travel time to and from work (to/from your place of residence) however, if you are driving with your supervisor or other staff member for work purposed (i.e., on a home visit or assisting clients off site), then these hours can be included.

Logbooks which fail to detail all activities undertaken and hours taken on the task may result in you having to complete additional hours of placement and you may receive a not yet competent assessment for your work placement unit.
