Practical Assessment 1

 For this task you are required to provide care and deal with three (3) clients with mental health issues in different situations where the clients’ behaviours are of concern, at least one (1) of the clients must have complex trauma and attachment issues and needs.

 If you are completing this in a simulated environment the client profiles and scenarios can be found at the end of this book.

 You are required to deal with the situation in accordance with operational policies and guidelines and ensure you use a trauma informed care approach, using effective communication techniques that engage with your client positively and supportively. You will also be required to complete a Client Report for each occasion and a short self-reflection on how you handled the situation and what you feel you could improve on next time.

 Your Assessor will be using the Observation Checklist (found after the Client Report template in this document) during your assessment and may ask questions from you during the assessment to further clarify your knowledge and understanding of tasks. Ensure you read through this checklist prior to your assessment so you are aware of what the Assessor is assessing you on.

 The Client Report templates are included below. It is highly recommended that you read through each section of the template and the Assessor’s Observation Checklist, so you know what you need to address.

 If you are completing it in your own workplace, you can use the letter template at the end of this document to gain client consent. Just copy, cut, and paste it into a Word document and add the client details. It may also give you a better idea of which clients you can work with and write about – as some clients may not address all the criteria in the template. Complete a separate report for each client.

Clients, their families and friends, and other staff must always be de-identified.

Client Report 1

1.Describe the history and trauma of the client and their daily needs, including identifying the indicators of trauma and/or attachment disorder.

2.Describe what happened during the shift and behaviours the client displayed while in your care.

3.Describe how you handled the situation using a trauma-informed care approach, the strategies and interventions you implemented to defuse the situation. (Including information on how you communicated and negotiated with the client).

Self-Reflection 1

Reflect on how you handled the situation and describe what you felt, went well, went poorly and what you would do in a similar future situation.
