Scenario


Allan is a 63-year-old widower (his wife died 10 years ago at age 50 from septicaemia). He lives alone in the marital home on a property on the outskirts of a rural town. He has a small number of farm animals and a number of pet dogs. Since his wife died, he has reported that he is unable to sleep well and has been prescribed tranquilizers to use at night. He recently broke several fingers on his left hand trying to load logs onto his truck. He attended emergency where a cast was set. He attended his local doctor who have him a script for pain killers. (codeine based).


Allan has groceries delivered to his farm once a week and this includes two cases of wine and 2 bottles of scotch most weeks. It is not clear if his medical practitioner is aware of the level of alcohol use however it is common knowledge to his family and friends.


Recently, it was noted by a family member that Allan was not answering his telephone. They requested a police wellness check. When the police arrived at the home, they found Allan to be in a confused and lethargic state. There was no evidence of food or self-care for what may have been up to a week. He appeared to be physically fragile. Allan was transported to the local rural hospital, treated with rehydration overnight and was eating well the next day. As there were no facilities available for long stay, he was sent home with a Blue Care referral. He also agreed to a home visit from your organisation, Recovery Unlimited, for an assessment and discussion on help available to him.
