Scenario 
Imagine you are Frank's care worker, that's Sarah In the videos. and you have been on annual leave and you haven't seen Frank for three weeks. In reviewing his case file, you notice he hasn't attended the Balance Croup for last two weeks. His first absence was due to it falling on a public holiday and Last week he told the driver who came to collect him that he didn't feet welt - he thought he might be coming dawn with something, maybe he had caught it from his niece who had the flu. 
You turn up at his home at your usual time, but Frank doesn't respond to your door knocking. You try knocking louder and you can hear his dog barking. You hear Frank responding to the dog and you call his name and then knock on the window. Eventually Frank comes to the door and you observe that he looks different. He looks dishevelled and appears to have lost weight. His pyjama top and bottom don't match, he has stains on his dressing gown, and he smells of stale urine. He is clutching a photo frame. He seems a little confused and you need to prompt him to remember who you are. 
You say to Frank: Hello Frank it' been a few weeks since you Le seen me. I was on holiday - I hope you have been alright. Can I come in? 
Frank shuffles back and allows you In. The house seems dark and you realise that most of the curtains are still drawn, which is very unusual. Frank doesn't talk to you and just moves to his chair, sits down and stares at the photo frame. His dog sits close to him and tries to get his attention, but Frank seems not to notice. 
You say. Frank how about we get you sorted with your shower and then we can have a cup of rea and you can tell me what you have been up to while I've been away? 
Frank gets up slowly and makes his way to the bathroom without talking. You follow him and notice that his home doesn't look as tidy as it normally does and that the area in the kitchen where he feeds the dog looks dirty, as if food has been spilt and not cleaned up. 
In the bathroom Frank needs to be prompted to go through the process of showering and you need to provide more hands-on assistance than usual for him to complete the task. After he has showered, you prompt him to get dressed and then he walks back the lounge room and he sits down, again holding the photo frame. while you are making his cup of tea you notice that his medication dosette box Is open and it looks like he hasn't taken his pills correctly. You bnng the tea to him with a biscuit. He doesn't seem to notice you and seems to be talking to the photo frame which you see contains a picture of his sister, Nancy.
