Problem/ Situation : The situation highlights gaps in metabolic and physical health monitoring for patients on clozapine therapy ,which is essential for mitigating the drugs serious side effects and improving treatment outcomes.Inadequate physical assessments during monthly clozapine clinic visits, stemming from insufficient nursing skills and lack of standardized protocols during clinic visits .Also patients nonadherence and -non-compliance with monitoring requirements .

Impacted stakeholders:

1)Patients:increased risk of premature mortality and reduced Treatment Efficacy

2. Healthcare Providers:poorer clinical outcomes and increased workload, and potential legal repercussions
3. Families and Caregivers:Emotional Burden and increased care responsibilities.
