In this assessment task you are provided with a scenario and relevant patient information.
Task Description
In this assessment task you are provided with a scenario and relevant patient information.
The person is Leon Tremblay. You have met Leon in the second intensive workshop. There is more information about Leon on this module page "Assessment task 3: the person".
You will research and apply clinical reasoning to understand the patient's presentation and then explain the rationale for therapeutic management of this patient.
This individual assignment will require you to submit a5-minute video recordingto demonstrate;your understanding of this patient's condition and the therapeutic management
your ability to communicate health information to fellow health professionals
Within the task you will be required to use appropriate and accurate discipline specific language.
IN YOUR OWN WORDS, you will need to:
Explain the rationalefor therapeutic approaches for this patient as identified on this module page "Assessment task 3: therapies for discussion".
Relate the therapeutic approachesto the pathophysiological presentation of this patient.
Use appropriate terminologyand communication style.
Assessment criteria and marking rubric
Your submission will be assessed against the following criteria. You are strongly encouraged to review the marking rubric for this assessment task as it provides more detailed information about the assessment criteria.
Assessment Criteria Measures IntendedLearning Outcome:
Criterion 1 Explain the pathophysiological basis for the patients health condition with reference to relevant anatomical structures and physiological processes. ILO1, ILO2 and ILO3
Criterion 2 Explain the rationale for therapeutic approaches. ILO2 and ILO3
Criterion 3 Use appropriate terminology to explain the patients health condition and rationale for therapeutic approaches. ILO3
Marking rubric
Carefully review themarking rubric for assessment task 3.
Markers will be assessing your work against the assessment criteria in this rubric.
Assessment due dates and submission details
Assessment task due date: Friday 22 March 2024, 15:00 hours (3pm)
Assessment weighting: 25% of your overall unit grade.
Task length: 5-minute video recording.
Submission details: Submit your final piece of work by the due date to Assessments>Assignments>Assessment Task 2 (see the page in this module "Submitting a video for assessment")
Other important information
Take careful note of the following before submitting your assessment task:
Task format and IMPORTANT details
5-minute video+/- 10% time lee way (4 min 30 seconds - 5 minutes 30 seconds) - any material after 5 minutes 30 seconds will not be marked. you may be asked to re-submit.
You MUST record the video inyour own voice.
You must not speed up the audio.
You mustidentify yourself and show your ID cardat the beginning if the recording. This does NOT count towards the time limit.
You mustshow your faceduring the entirety of the recording.
Supporting technology such as PowerPoint slides are NOT required.
Only provide a VERY brief introduction of the patient (we suggest using only the first 30 seconds)
Note: Please don't just read a script placed right behind of the camera. That is not appropriate communication. Explain the content to the camera as if you were talking to a colleague. A few humms and aaaahs and briefly looking down to notes are more natural than reading a speech meant to be read, not listened. You may record the video in chunks; it does not have to be in one long take. But please speak to the listeners, don't read to the listeners.
Referencing and guidelines
Referencing is not required, however, you are required to identify the guidelines which support the therapy/interventions suggested for Leon. Guidelines, policies and procedures are an important part of rationale for therapies and interventions.
Extensions
Extensions will be offered according to theSchool of Nursing Assessment Guidelines. You can also find these guidelines in the Assessment help and resources section on MyLO. This states that unless for medical or other documented reasons, extensions must be requested prior to the deadline.
Please note that Guidelines 5.1 and 5.2 will be strictly adhered to.
Ensure that you send your requestand supporting documentationtoCXA243@utas.edu.au. Note that being on PEP or work commitments are NOT grounds accepted by the School of Nursing to grant extensions.

Leon Trembley
Leon Tremblay is 67 yo. He presented to the clinic and after having an HbA1c test (result 9%) was diagnosed with type 2 Diabetes mellitus.
His cardiovascular risk was > 15%http://www.cvdcheck.org.au/His renal health screen showed an eGFR 90 mL/min/1.73m2with microalbuminuria of 3.5 mg/mmol.
http://www.kidney.org.au/HealthProfessionals/DetectingCKD/tabid/632/Default.aspxHe attended a podiatry appointment which detected decreased dorsalis pedis and posterior tibialis pulses in both legs .
Management of his condition now includes the following:

Ongoing diagnostics:
HbA1c: every 3 months
Lipid profile: every 3 months

Podiatrist assessment: at least once a year
Optometrist assessment: at least once a year

Context of the presentation
While caring for Leon as a nurse, you have been asked to present his case to your colleagues as part of an in-service about type 2 Diabetes mellitus.
What is in service training ?
In-Service training in nursing is a form of continuing professional education provided to nurses during their employment. It comprises logically planned educational activities aimed at enhancing the employees' performance, thereby improving service quality. This training keeps nurses updated with the latest developments in their profession and is crucial for improving patient care quality. Techniques such as case-based learning, clinical simulations, and repetitive interventions are often used.

Your presentation will focus on the therapies and interventions for the ongoing management of Leon's condition. In all, you need to chooseone therapy and one interventionfrom the lists below, about which you will educate your colleagues.
Your presentation needs to demonstrate that you are speaking to your colleagues IN YOUR OWN WORDS. You can, as you would in an education session such as this, use notes, but you would not read everything from a script. You do NOT need to use a PowerPoint presentation. You will be successful with this assessment if you demonstrate that you are explaining concepts using your own words.
A. From this list chooseONE (1)therapyto discuss:
empagliflozin
metformin
rosuvastatin
ramipril
You need to do the following for each medication:
describe the mechanism of ACTION/S
identify the AIM of the therapyforLeon(i.e. specific for this patient)
relate the therapy to Leon's pathophysiological presentation
refer briefly to any GUIDELINES which support the use of the medication
identifyoneRISKor special consideration for Leon whilst taking the medication
identify what evidence will demonstrate the EFFECTIVENESS of the therapy if you wereevaluating careprovided to Leon
B. From this list chooseONE (1)therapeutic interventionfrom either of the following groups, to discuss:
Group 1:
If you choose one of this group, you need to identify the AIM of this intervention (i.e.whyis Leon recommended to do it)by discussing the intendedphysiological effectsspecific to Leon (i.e.,howthe intervention causes the beneficial effects to Leon). You need to refer briefly to any GUIDELINES which support this intervention.
exercise plan
smoking cessation plan
alcohol consumption plan
dietary planning
OR
Group 2:
If you choose one from this group, you need to identify the AIM of this intervention (i.e.whyis Leon recommended to do it) by discussing whatpathophysiological changes, specific to Leon, that the intervention is intended to detect. You need to identify whathealth impactthose pathophysiological changes might have forLeon. You need to briefly refer to any GUIDELINES which support this intervention.
yearly optometrist assessment
yearly renal function assessment
