• Explore the prevalence, multi-morbidity and severity of recognised LTC from a biological and psychosocial perspective
• Give an outline and rationale for LTC chosen including how altered physiology impacts on the person’s physical body.
• Indicate prevalence of the LTC including statistics and long term health outcomes.
• Discuss and appraise how psychosocial issues can impact on the health and wellbeing of this chosen group including motivation and locus of control

• Analyse critically and apply evidence based practice when assessing, planning and managing long term conditions.
• Discuss and appraise current evidence for the LTC including appraisal of opportunities and challenges presented to nurses when assessing, planning and managing the LTC. Explain how the biopsychosocial elements may impact on this.

• Explore the attitudes, values and beliefs which may impact upon self-monitoring and self-management of long term conditions.
• Identify and explore aspects of attitudes, values and beliefs related to the LTC. This could include cultural, ethical, legal or safeguarding. This may include producing resources in other languages or consideration of where / how to use the resource.

• Examine the nurse’s role in supporting, promoting and protecting the well-being of individuals living with a long term condition.
• Consider how nurses will work collaboratively with individuals and members of the multi-disciplinary team to support, promote and protect wellbeing of the person with a LTC. Consider evidence based pathways to support this (e.g. NICE)

• Develop a resource which promotes health and well-being based on collaborative principles
• Identify a model(s) / framework(s) utilised in self-management related to the chosen LTC. Give a clear discussion of the possible ways of applying the model(s) / framework(s) to the selected topic and the resource created.
• Design and develop a resource (for example web page, poster, academic game, workbook etc.) to increase awareness or which supports a long term condition (LTC)
