When and where will this assessment take place?
This assessment may be undertaken in a simulated environment/in a classroom/at home to be discussed with the assessor.
Your assessor will provide you with the due date.
Your assessor may also specify the length of time allowed and the expected word count, if applicable.
What if the assessment is not suitable?
If you are unable to provide sufficient evidence within the assessment task, your assessor may be able to provide you with an alternative method. Discuss this with your assessor.
What happens if your answers are not satisfactory?
If any of the assessment is not satisfactory, your assessor will provide you with feedback. You may need to resubmit some or all the assessment tasks. Your assessor will explain the details of your reassessment.
In most cases, you will be allowed a maximum of three attempts.
If you are not satisfied with your assessment result, you have the right to lodge an appeal. Ask your assessor or contact the Course Coordinator for more information.

Assessment Instructions: Creating a Privacy and Confidentiality Policy
This assessment has two parts.
Part 1: Create a Policy.
As a key member of the Equality First team, your manager has asked you to write a privacy and confidentiality policy. Equality First has recently developed a procedure document for privacy and confidentiality in their community services organisation (see below). They also have a template that they use to record their policies (see below). The template provides a guide to the word count for each section. This word count guide is to help you understand how much information each of your policy sections should contain.
Your task is to create a policy document that complements the practices and procedures of the organisation and follows the policy format that we have covered in this unit (see your learner guide for more information). Read the followingPrivacy and Confidentiality Procedurethat Equality First has documented and then write a policy by completing the below template.
Part 2: Reflective Question
Write a short reflection on why your policy is important to the service users. This is a short statement that asks you to provide your own views on why you think that the policy that you have written is important.
Privacy and Confidentiality Procedure
The following procedures comply with the Australian Privacy Principles (APP's) in the Privacy Act 1988 and related Regulations.
Collection of Information
Equality First (EF) only gathers personal information for the purpose of offering services that are directly linked to the organisation's objectives. This personal information may include details about a person's biological, psychological, and social circumstances. Additionally, EF will gather contact information from suppliers to place orders on behalf of the organisation. EF will also gather personal information from employees relevant to their employment, as well as from volunteers who are serving the organisation, and from contractors who are engaged to perform work for the organisation. All information will be collected on standard forms including the staff information form and the service user intake/assessment form.
Use and Disclosure
Equality First (EF) will not disclose any personally identifiable information to any third party unless it is required by law. In cases where personal information needs to be disclosed to another person, EF will obtain prior consent from the individual, for example, to verify employment with a bank or to meet legal obligations using the consent to disclose information form. In the event of an emergency where disclosing personal information about an individual is necessary to prevent harm or injury to another person, EF may provide relevant personal information. However, in such cases, a record of the situation and decision-making will be included on the critical incident report form, including to whom the information was released.
EF will not use personal information or images for direct marketing, social media, or the organisation's website unless written consent is obtained beforehand and an opt-out option is provided. If employees or volunteers are unsure whether the information is confidential, they should refer to the organisations management before disclosing any information to an external source.
Access to Personal Information
Employees, volunteers, contractors and service users may access their personal information at any time by contacting management.
Security of Information
Equality First (EF) will take necessary measures to safeguard personal information against misuse, loss, unauthorised access, interference, modification, or disclosure. These measures include reasonable steps to secure both electronic and hard-copy records, such as locking filing cabinets and storage areas, physically securing areas where personal information is stored, not storing personal information in public areas, establishing different access levels to limit information access to employees and volunteers, installing virus protections and firewalls, and positioning screens of computers, phones, and tablets in a way that prevents unauthorised access.
EF will dispose of personal information when it is no longer needed for the purpose for which it was collected, including information stored on decommissioned laptops and mobile phones.
Confidentiality
All individuals associated with Equality First (EF) have an obligation to maintain confidentiality and not disclose or discuss confidential information with others, even after their association with EF has ended. During their employment, service, or engagement with the organisation, individuals may come across sensitive information regarding EF or its service users, which must be kept confidential.
EF expects that all individuals treat any information acquired during their association with the organisation as confidential, including but not limited to commercially sensitive or confidential information, sensitive information such as financial transactions and personal information of service users accessing services, the information provided by current or potential service users, and any other information indicated as confidential from time to time.
Breaches
If any individual suspects a breach of confidentiality or privacy, they should report the matter to the management of Equality First (EF) using the Breach of Standards form. Breaches of this policy may lead to disciplinary action, which will depend on the severity of the breach. The action taken may include counselling, mandatory training, a formal warning, or termination of employment or contract.
