- This document contains two (2) assessment methods used to determine competency for the unit. They include practical tasks and knowledge questions.
- To achieve a satisfactory result in this assessment, all parts of this document must be filled in by either answering the question, performing the task, or providing workplace evidence.
- Knowledge questions must always be completed and cannot be replaced with workplace evidence.
- If completing on hardcopy, you must use blue or black pen only. Whiteout, erasable pens, pencils, or other coloured pens are not acceptable.
- A degree of independent research utilising current and reliable sources may also be required to complete the assessment.
- Please ensure you keep a copy of your assessment for you own records. If your assessment is lost, it is your responsibility to provide a copy or reproduce your work as required to demonstrate satisfactory practical knowledge.
- The outcome of each practical task or knowledge question is either Satisfactory (S) or Not Yet Satisfactory (NYS).
- You have three (3) attempts to complete each practical task and knowledge question.
- Please note that this is an open book assessment. The Study Guide for this unit may be used to assist you.

Appeals
You have a right of appeal if you are not satisfied with the outcome of your assessment.

Practical Assessment Methods
Some of the practical assessments for this unit may be completed by the following methods:
- Providing workplace evidence: The workplace evidence must meet the performance requirements as stated in the practical task.
OR
- Performing the task: In case you cannot provide workplace evidence you may perform the task.
OR
- A combination of assessment methods may be used.
Evidence
Look for evidence that meets the requirements for multiple tasks. If you can provide evidence that covers several tasks it will minimise time spent searching for and compiling multiple forms of evidence.
Evidence must also meet the following criteria:
- Validity:Evidence must show that you have the skills, knowledge, and attributes to be deemed competent in the unit of competency.
- Sufficiency:The quality, quantity, and relevance of the evidence you submit must the demonstrate your competency.
- Currency:We prefer current proof of competence so please submit recent documentation as evidence. As a rule of thumb, evidence from the last two (2) years is preferable. Discuss this with your Assessor. Academic training and qualifications older than two (2) years may be submitted to demonstrate knowledge of vocational education and training.
Practical tasks
The practical tasks for this unit comprise a combination of assessment tasks. These include:
- Performing work safely
- Monitoring work groups
- Managing and reporting risk
- Communicating and consulting a work group on WHS issues
- Supporting the implementation of participative arrangements
- Investigating of hazardous incidents
- Demonstrating your knowledge of emergency procedures.
Practical Task 1: Perform all work safely |
Task description: This task requires you to perform all work safely. Tasks may include test work, calibrations or maintaining equipment. Task instructions: Follow your organisational policies and procedures to safely perform test work, calibrations or maintaining equipment in the presence of your assessor. Use established work practices and personal protective equipment (PPE) to ensure personal safety and that of other laboratory personnel Clean, care for and store equipment, materials and reagents as required Minimise the generation of wastes and environmental impacts Ensure safe disposal of laboratory/hazardous wastes Ensure hazard controls and PPE appropriate to the work requirements are available and functional Your performance must meet the criteria listed in the following Evidence Checklist. |

Task description: This task requires you to participate in risk management processes. Instructions for the student: Provide evidence that you have participated in risk assessments to identify and analyse risks, including: Reported and addressed any identified hazards and inadequacies in existing risk controls within level of responsibility and according to workplace procedures Supported the implementation of procedures to control risk (based on the hierarchy of control) Participate in risk assessments to identify and analyse risks. Evidence must show you have performedthe above. Evidence must meet the criteria listed in the following Evidence Checklist. If you are unable to provide sufficient evidence, you must complete the following task. Task instructions: Follow your organisational policies and procedures to participate in risk management processes: Do a walkthrough of your work area looking to identify existing hazards. Complete the Risk Assessment Report provided by: oListing each hazard identified (to a maximum of 4 hazards) oAssessing the risk of each hazard oAssessing existing control measures for inadequacies (to a maximum of 2 controls) oIf inadequate, address the hazard (within your level of responsibility and according to workplace procedures) Draft an email to your supervisor including the completed Risk Assessment Report asking for acknowledgement of the report. Task description: This task requires you to effectively monitor WHS within 1 work area. Instructions for the student: Provide evidence you have effectively monitored WHS within 1 work area, including: Maintained work health and safety Ensured others in the workgroup work safely and followed procedures for hazard identification and risk control and implement safe work practices Ensured workplace procedures were clearly defined, documented and followed Identified any deviation from identified procedures and reported and addressed within your level of responsibility Identified and addressed training needs within your level of responsibility. Evidence must show you have performed the above. Evidence must meet the criteria listed in the following Evidence Checklist. If you are unable to provide sufficient evidence, you must complete the following task. Task instructions: Follow your organisational policies and procedures to monitor and maintain WHS within one (1) work area: Complete the Monitoring form provided to: oEnsure workplace procedures are clearly defined, documented, and followed oEnsure others in the workgroup work safely and follow procedures for hazard identification and risk control and implement safe work practices oIdentify any deviation from identified procedures and address within your level of responsibility Use the Training Needs Analysis form to identify and address training needs within your level of responsibility Email the completed Monitoring form to your supervisor to report the task outcomes. Submit all evidence to your assessor: oCompleted Monitor form oEmail to your supervisor. We encourage you to use your workplace templates and systems to complete this task, however, if these are not available, you may use the templates provided. |

Training Record
This Training Record is an important official record of the training a Trainee will receive during the course of obtaining a nationally recognised qualification.
The Training Record lists all the units of competence, performance criteria and workplace tasks a Trainee must successfully undertake in order to complete the qualification.
Trainee Responsibilities
- Undertake all training tasks as requested by the workplace supervisor or LTT Trainer/Assessor
- Ask for assistance if required contact your LTT Trainer/Assessor
- Acknowledge ability to perform workplace tasks.
Employer Responsibilities
- Provide adequate and suitable training experiences to enable the Trainee to attain competence
- Ensure that the Participant is supervised and guided at all times during the training period
- Acknowledge competence in the workplace to industry and company standards by reviewing the Training Record and signing to verify on a regular basis no greater than 3 months
- Advise LTT if any training issues arise
- Trainee must be withdrawn from routine work duties to undertake structured training for a minimum of 3 hours (VIC).
How to use your Training Record
- This Training Record contains all the competencies that relate to the skills and knowledge you (the Participant) have to demonstrate in a practical way in the workplace.
- You have to show you can perform the competencies in this Training Record in a workplace setting. When you can, you will be considered competent at those particular tasks.
- The units of competency are divided into smaller elements and performance criteria that is, specific tasks you will be asked to show you can do. You must be observed in your workplace and assessed as being competent by your Trainer/Assessor, assisted by your Workplace Supervisor and any other relevant observer.
- Assessment is not graded. It is either: Competent or Not Yet Competent
- To be considered competent, you need to demonstrate skills: consistently, and to the standard required.
- Your Workplace Supervisor and your Trainer/Assessor will both be involved in teaching and assessing you in the workplace throughout your traineeship.
- If you do not keep up the expected standard after being assessed as competent, youre Workplace Supervisor and your Trainer/Assessor can, in consultation with you, reverse the assessment after you have been notified
- It is the Participants responsibility (your job) to check that all competencies have been signed off by your Trainer/Assessor and Workplace Supervisor before you submit your Training Record to your Trainer/Assessor for final assessment.
