Mr John is a 89 years old male presented to hospital after he had a fall from home. Mr John had a history of heart failure, dementia, and Type 2 diabetes. I was assigned with my buddy nurse to the care of Mr John. Due to high risk of fall Mr John was on high visibility room, on proximate alarm to alert the nursing staff and he ambulated with 4WF with supervision. Mr John was noncompliant with nursing care he often gets angry when we approached him to check his vital signs, blood sugar and supervised him to the toilet. During PM shift when I walked into his room to introduce myself to the patient his room was smelled like faeces and when I checked his pant was wet. I asked Mr. John if I can take him to toilet to change his pad and Pant. He refused to change his pad and told me to go away. I explained to Mr. John why the need to go toilet, but he refused. As a student nurse I reported my concern to my buddy nurse and my buddy nurse told me to ignore him as he would not let me change his Pad. Mr. John have a cognitive impairment with Glasco coma scale (GCS) 14 due to that it is hard to understand the consequence of not changing soiled pad and make right decision. I was in ethical dilemma do I have to respect his wish and just ignore him as my buddy nurse said or do, I have keep explained to him until Mr John agreed to change his pad and cloth. I could not just ignore him, so I keep tried to be explained to Mr john, but he keeps refusing and told me to go away. My buddy nurse did not seem concerned at all as she thinks he is stubborn, and she did not even try once to talk to him. During afternoon break my buddy nurse left me to look after our allocated patients under supervision of the other Registered nurse. I was very concerned, and the nurse asked me if I am ok. I explained to the nurse about my concern, and we went together to Mr Johns room and the nurse told him to change his pad as it is very wet after a lot of prompting he agreed to change it himself under supervision.
