The MSE provides a mostly safe foundation for patient care that with could provide some useful information to the multidisciplinary team.
30-34.5 Multiple errors/omissions in MSE/MSE lacking to the extent that patient care may be compromised, and/or information provided has some irrelevant/inaccurate content and/or there are some omissions/inaccuracies in observation/interpretation of information and/or acknowledgment/understanding of patient cues and their significance is limited. MSE provides some information that could be useful to the multidisciplinary team.
25-29.5 Significant errors in MSE/MSE lacking to the extent that patient care would be compromised and/or information provided is mostly irrelevant/inaccurate and/or there are significant omissions/inaccuracies in observation/interpretation of information and/or lack of acknowledgment/understanding of patient cues and their significance. MSE is of limited to no value to multidisciplinary team.
