  2. To achieve a satisfactory result in this assessment, all parts of this document must be filled in by either answering the question, performing the task, or providing workplace evidence.

Practical Assessment Methods

Some of the practical assessments for this unit may be completed by the following methods:

  • Providing workplace evidence: The workplace evidence must meet the performance requirements as stated in the practical task.

OR

  • Performing the task: In case you cannot provide workplace evidence you may perform the task.

OR

  • A combination of assessment methods may be used.

Evidence

Look for evidence that meets the requirements for multiple tasks. If you can provide evidence that covers several tasks it will minimise time spent searching for and compiling multiple forms of evidence.

Evidence must also meet the following criteria:

  • Validity:Evidence must show that you have the skills, knowledge, and attributes to be deemed competent in the unit of competency.
  • Sufficiency:The quality, quantity, and relevance of the evidence you submit must the demonstrate your competency.
  • Currency:We prefer current proof of competence so please submit recent documentation as evidence. As a rule of thumb, evidence from the last two (2) years is preferable. Discuss this with your Assessor. Academic training and qualifications older than two (2) years may be submitted to demonstrate knowledge of vocational education and training.

Practical Task 1: Receive and log laboratory samples

Task description:

This task requires you to safely and accurately receive and register samples to maintain sample integrity and traceability.

Instructions for the student:

Provide evidence you have safely and accurately received and registered six (6) samples to maintain sample integrity and traceability, including:

Addressed client and customer service issues.

Evidence must show you have performed the above.

Evidence must meet the criteria listed in the following Evidence Checklist.

If you are unable to provide sufficient evidence, you must complete the following task.

Task instructions:

Follow yourorganisational policies and procedures to safely and accurately validate and log six (6) samples into a LIMS in the presence of your assessor, then:

Check and match samples with request forms before they are accepted

Use the email template provided to email a client to report that samples and/or request forms do not comply with workplace requirements.

We encourage you to use your workplace templates and systems to complete this task, however, if these are not available, you may use the templates provided.

Training Record

This Training Record is an important official record of the training a Trainee will receive during the course of obtaining a nationally recognised qualification.

The Training Record lists all the units of competence, performance criteria and workplace tasks a Trainee must successfully undertake in order to complete the qualification.

Trainee Responsibilities

  • Undertake all training tasks as requested by the workplace supervisor or LTT Trainer/Assessor
  • Ask for assistance if required contact your LTT Trainer/Assessor
  • Acknowledge ability to perform workplace tasks.

Employer Responsibilities

  • Provide adequate and suitable training experiences to enable the Trainee to attain competence
  • Ensure that the Participant is supervised and guided at all times during the training period
  • Acknowledge competence in the workplace to industry and company standards by reviewing the Training Record and signing to verify on a regular basis no greater than 3 months
  • Advise LTT if any training issues arise
  • Trainee must be withdrawn from routine work duties to undertake structured training for a minimum of 3 hours (VIC).

How to use your Training Record

  1. This Training Record contains all the competencies that relate to the skills and knowledge you (the Participant) have to demonstrate in a practical way in the workplace.
  2. You have to show you can perform the competencies in this Training Record in a workplace setting. When you can, you will be considered competent at those particular tasks.
  3. The units of competency are divided into smaller elements and performance criteria that is, specific tasks you will be asked to show you can do. You must be observed in your workplace and assessed as being competent by your Trainer/Assessor, assisted by your Workplace Supervisor and any other relevant observer.
  4. Assessment is not graded. It is either: Competent or Not Yet Competent
  5. To be considered competent, you need to demonstrate skills: consistently, and to the standard required.
  6. Your Workplace Supervisor and your Trainer/Assessor will both be involved in teaching and assessing you in the workplace throughout your traineeship.
  7. If you do not keep up the expected standard after being assessed as competent, youre Workplace Supervisor and your Trainer/Assessor can, in consultation with you, reverse the assessment after you have been notified
  8. It is the Participants responsibility (your job) to check that all competencies have been signed off by your Trainer/Assessor and Workplace Supervisor before you submit your Training Record to your Trainer/Assessor for final assessment.
