1) Present a formulation of the service user, using a recognised model, which demonstrates how data derived from assessment explains the causative factors of the services users mental health problems.
2) Demonstrate the ability to apply and explain the use of biological, psychological and social interventions with this service user, incorporating risk management, multiprofessional collaboration, person-centred care planning and carer involvement.
3) Give an analytical account of how the effectiveness of care for this service user was assured and evaluated. Refer to multi professional and multi-agency involvement and recognised evaluation frameworks.
4) Analyse any legal and ethical issues and unmet needs and how these were addressed by the service and organisational processes.
The case report needs to be supported with references of evidence-based practice, policy, guidelines and legal and ethical frameworks. A reference list needs to be included at the end of the case report.

Formulation has been defined as synthesizing the patient's experience with relevant clinical theory and research and is the bridge between assessment and treatment.
Phenomena such as mood and personality disorders, psychoses, and anxiety disorders can be associated with a diversity of variables

Biopsychosocial model
According to the biopsychosocial model, interactions between people's genetic makeup (biology), mental health and personality (psychology), and sociocultural environment (social world) contribute to their experience of health or illness.
The biological influences on mental health and mental illness are varied, and include genetics, infections, physical trauma, nutrition, hormones, and toxins.
The psychological component looks for potential psychological explanations for a health problem, such as lack of self-control, emotional turmoil, or negative thinking.
Social and cultural factors are conceptualized as a particular set of stressful events (being laid off, for example) that can differentially impact mental health depending on the individual and his or her social context.
The biopsychosocial theory posits that each one of these factors is not sufficient to create health or mental illness, but the interaction between them determines the course of one's development.
Despite its usefulness, there are issues with the biopsychosocial model, including the degree of influence that each factor has, the degree of interaction between factors, and variation across individuals and life spans.

Address psychological interventions e.g. CBT, Psychosocial interventions.

Revisit the identified goals and present the evidence which demonstrates whether or not these have been met.
Include the evidence presented by the client, their carers and the members of the team.
Make reference to frameworks such as CPA, or relevant care pathways.
