Ivan is a first year Student Nurse (SN) on his clinical placement for his nursing degree.HisplacementisatThomastownResidentialAgedCareFacility(TheRACF). Ivan has been assigned to work with Naomi, a Registered Nurse (RN), for todays morning shift that starts at 0700 hours.
IvanandNaomicommencetheirmorningshiftbyintroducingthemselvestotheir ten (10) allocated patients. They then proceed to plan care for each patient for their8-hourshift,bylookingthroughthefilestoprepareeachpatientsdailycare plan. They commence attending to each patients cares, from Bed 1 to Bed 10.
By 1100 hours, Ivan notices that one of the patients, Elsa, an 80-year old female, has soiled herself in her bed. Ivan lifts Elsa from her bed, helps her to a chair and cleansthebed.HethendocumentsthisinElsasclinicalnotes,forNaomitoreadat a later stage.
At1500hours,NaomiinformsIvanthatsheneedstoleaveearlyasshehasahair appointmentscheduled.NaomirequeststhatIvancompletetherestofthecares required for the ten (10) patients, for the last hour of the shift, on his own. Ivan agrees.
