IntroductionWelcome to the Student Assessment Tasks for CHCAGE001 Facilitate the empowerment of older people. These tasks have been designed to help you demonstrate the skills and knowledge that you have learnt during your course.
Please ensure that you read the instructions provided with these tasks carefully. You should also follow the advice provided in the Community Services Works Student User Guide. The Student User Guide provides important information for you relating to completing assessment successfully.
Assessment for this unitFor you to be assessed as competent, you must successfully complete three assessment tasks:
Assessment Task 1: Knowledge questions You must correctly answer all questions.
Assessment Task 2: Role play You are required to participate in a simulated interaction with an older person.
Assessment Task 3: Portfolio You must complete your portfolio document to demonstrate that you responded to the goals and aspirations of one older person.
Work placement required
You will require a work placement to complete Assessment Task 3. You will find more information about the requirements for work placements in the Community Services Works Student User Guide.

Role plays are designed so you can demonstrate the skills and knowledge which you have acquired during the learning phase of this unit.
This role play is to be completed in the simulated workplace environment.

This assessment task requires you to respond to the goals and aspirations of an older person in a simulated environment where you will need to demonstrate:
employing flexible, adaptable and person-centred approaches to empower the person
recognising and responding appropriately to situations of risk or potential risk
using oral communication skills to maintain a positive and respectful relationship with the person.
Prepare for the interaction.
Access the CHC Simulation pack and, along with your assessor, select a Client Profile on which to base this assessment task. Make sure you are familiar with the background of the case study workplace and the client profile you have selected. Your assessor will be role playing this person for this assessment.
Open and read through your Role Play Submission Document for this assessment task. Complete Section 1 of the document and read through Section 2 as preparation for the role play.
Participate in the role play with your assessor.
Begin by introducing yourself and confirming the context of the interaction.
Ensure the client that you will maintain confidentiality about what is discussed, in line with the Privacy Policy and Procedures.
Ask them if they mind you taking notes during the meeting. You will need the notes to complete the assessment. Use Section 2 of the Role Play Submission Document as the basis for your discussion.
During the meeting, you are required to demonstrate the following:
You can develop a relationship with the client that promotes trust
You can get them to talk about ageing and their quality of life
You can encourage an active ageing approach and empower them to have a shared responsibility for their quality of life
You respect their social, cultural and spiritual ideas and interests
You can discuss complaints mechanisms with them and ensure they understand the information you provide if they would like to make one
You listen to them and are alert for any areas of concern. Discuss the options that can be taken and support the person to access these supports and options.
During the interaction, you will need to demonstrate effective communication skills including:
Speaking clearly and concisely
Using non-verbal communication to assist with understanding
Asking questions to identify required information
Responding to questions as required.
Using active listening techniques to confirm understanding.
Record meeting details and plan for service delivery changes
After the meeting, complete Section 2 and Section 3 of the Role Play Submission Document.
Section 2 requires you to record the notes from your interaction into a neatly compiled record and Section 3 requires that you action outcomes from the meeting and plan adjustments and changes to service delivery based on the discussions from the role play.
Report to your supervisor (your assessor) any items necessary as per the details in Section 3.
Check the document for neatness and accuracy and submit to your assessor.

Successful completion of this assessment task requires you to demonstrate your skills and knowledge by responding to the goals and aspirations of one older person. You must:
develop relationships with older people
provide services to older people
support the rights of older people
promote health and re-ablement of older people.
It is important that you provide evidence that you have completed this requirement and we have provided you with a Portfolio Template to work into.
You will collect most of your evidence and complete portfolio documentation during the work placement phase of your course. You may also complete some of your portfolio in the classroom. The rest can be completed in the classroom after your work placement is finished.
Your assessor may visit you in the workplace to observe certain aspects of this assessment and your workplace supervisor will also provide feedback.
For the purposes of this assessment task, you will require access to all the necessary facilities, equipment and resources to complete this task. You will also require access to your workplaces policies and procedures.
Remember, it is important to keep the privacy and confidentiality of the individuals you work with as high priority. Therefore, you must deidentify all the documentation and evidence that you submit with your portfolio according to your organisations policies and procedures. If you are unsure about how to do this, speak to your assessor or workplace supervisor.
Tips for completing your portfolio
Read through this assessment and your Portfolio Template before you get started and make sure you understand what you need to do. If you are unsure, speak to your assessor and/or workplace supervisor.
Stay up to date! Complete your portfolio entries as you go and ask your supervisor to do the same. Providing organised, complete evidence forms part of your assessment.
Stay in touch with your assessor. Ask questions, raise issues, check in, communicate.
Most importantly, ask for help if you are having trouble!
Important note: Throughout your portfolio, you are asked to provide documents as evidence of the work that you have done. If you have permission to make copies of the documents described in the portfolio template, include these as part of your evidence by attaching them at the end of the portfolio. If you are not allowed to remove any workplace documentation from your workplace, then your assessor will be required to look at these documents during a workplace visit.
You must make sure you receive permission to include any documents from your workplace and all private information must be redacted.

Use the Portfolio Template to collect evidence for this assessment during your work placement period.
There are three sections of your portfolio that need to be completed:
Section 1 About the older person
You will provide detailed information about the person you worked with and your role.
Section 2 Activities
You will describe the activities which you used to respond to the goals and aspirations of an older person and how you provided support services. Describe each activity and what you did in detail.
Section 3 Supervisor evidence
Your supervisor needs to complete this section once all of the tasks have been completed and relevant sections have been endorsed.

She has recently suffered an illness that has left her unable to walk far and has been given a wheelchair to use. On your first visit you complete a risk assessment as per the requirements of your workplace.
You observe that Mrs Keens house is very small, overcrowded with furniture and there is evidence of hoarding newspapers and magazines so moving a wheelchair through the house is not possible. You identify that the house has no smoke alarms or safety switch, a number of electrical cords are frayed, and the electric heater is covered in dust. Mrs Keen is also coughing and sneezing.

This assessment task requires you to apply workplace safety procedures during day-to-day work activities in a simulated environment where you will need to demonstrate:
assessing risks and recording the results
infection control
safe manual handling techniques
use of Personal Protective Equipment (PPE)
reporting incidents and hazards
that you can reflect on your own safe work practices in a debriefing meeting
following the procedures and instructions for a simulated emergency situation.
Prepare for the interaction.
Access the CHC Works Simulation Pack and, along with your assessor, select a Client Profile on which to base this assessment task. Before you begin, take some time to familiarise yourself with the background of the case study workplace and the client profile you have selected. Your assessor will be role playing the client for the purposes of this assessment.
Open and read through your Role Play Submission Document for this assessment task. Complete Section 1 of the document and read through Section 2 as preparation for the role play.
Read the scenario to prepare for the interaction.
You have been providing support to this client for some time but they had a fall recently which has left them with some short-term mobility issues and a large open wound on their leg. The open wound on their leg is infected and the client notes state that they have been diagnosed with VRE (Vancomycin-resistant Enterococci). VRE is highly is highly contagious.
Normally, your client can get themselves into the shower with little or no assistance but today you will need to use an assistive device to provide some additional support.
Due to the changes in your clients situation and the environment (manual handling and infection) you will need to complete a new risk assessment before you start work.
During the process of providing mobility support to your client, you will discover that the integrity of your glove has been breached and you suspect that fluid from the wound has been transferred to your skin. You will need to respond and take immediate action to rectify the situation and follow up by reporting the incident. You should follow the WHS policy and procedure manual to do so.
Your client will also display some challenging behaviours during your morning routine which you will need to respond to and report.
Participate in the role play with your assessor.
Begin by introducing yourself to your client and confirming the context of the interaction.
Ask them if they mind you taking notes during the interaction. You will need the notes to complete the assessment. Use Section 2 of the Role Play Submission Document as the basis for your discussion.
During the interaction, you are required to demonstrate the following:
That you can identify hazards and risks and complete a risk assessment prior to commencing work with the client. Work into the Risk Assessment Tool Template (or your workplaces/RTOs standard form) to do this.
You can report hazards in accordance with WHS policies and procedures. Work into the Hazard Identification Report Form Template (or your workplaces/RTOs standard form) to do this.
You can apply infection control procedures including donning and doffing personal protective equipment.
You can carry out hazardous manual handling tasks safely.
You can report incidents in accordance with WHS policies and procedures. Work into the Hazard Identification Report Form Template (or your workplaces/RTOs standard form) to do this.
Record interaction details and complete required documentation
After the interaction, complete Section 2 of your Role Play Submission Document.
Section 2 requires you to record the notes from your interaction into a neatly compiled record.
Complete all sections of the Risk Assessment form, Hazard Report form and the Incident Report form.
Check the documents for neatness and accuracy.
Participate in the debrief meeting with your supervisor.
Following what turned out to be a difficult interaction with your client, you have requested a debrief with your supervisor. Request a meeting with them (your assessor will play the role of your supervisor) and complete the following tasks:
Read Section 3 of the Role Play Simulation Document and make some notes for your debrief meeting.
Begin by introducing yourself and confirming the context of the interaction.
Ask the person you are meeting with if they mind you taking notes during the interaction. You will need the notes to complete this part of your assessment. Use Section 3 of the Role Play Submission Document as the basis for your discussion.
During the debrief, you are required to demonstrate the following:

you can reflect on your levels of stress and/or fatigue (in relation to the tear in the glove incident and the client behaving aggressively towards you) and how it is impacting your work or home life. You can imagine that the incidents have taken their toll and has affected work and home. Reflect on how this might be so for the purpose of the assessment
participate in a workplace debriefing to identify and address your needs.

Section 3 requires you to record the action required from your interaction into a neatly compiled record.

Participate in a simulated emergency.
Your assessor will arrange for a simulated emergency event. You will be required to follow directions and instructions given to you by the Chief Fire Warden and Fire Wardens.
After the simulation is over, complete Section 4 of your Role Play Submission Document. All questions must be correctly answered in this section.

This assessment task requires you to participate in a WHS meeting and to contribute to the development and implementation of a workplace policy and procedure for your work area. You will work with your assessor and at least one other person (a fellow student or colleague) to form a WHS committee.
You will need to demonstrate:

that you kept a record of the actions and outcomes of the meeting
that you contributed to the development of a WHS practice
that you contributed to the implementation of the developed practice.
Prepare for the interaction.
Access the CHC Simulation pack. Make sure that you are familiar with the background of the case study workplace. Your assessor will be role playing the WHS Committee Chairperson for this assessment.
Read the scenario to prepare for the WHS committee meeting.
Reports of an outbreak of an infectious disease have just started to surface in your area. Older Australians have been identified as an at-risk group and information has been released by authorities on how aged care services should proceed.
The Healthy Lives residents are at high risk and guidance from local authorities advises that the facility should implement the following strategies:
visitors should be limited to essential services (for example, medical staff, essential deliveries etc)
all staff must wear masks during their shift except for when they are eating
all common areas in the facility must be closed
standard PPE (masks, gloves, glasses and gowns) are to be worn whenever there is contact with residents
residents must stay in their rooms except for in limited circumstances (medical appointments etc).
Healthy Lives senior management has also directed the committee to implement tighter hand washing procedures.
You are on the WHS Committee and the chair has called a meeting to put urgent protocols in place for staff, residents and visitors.
Participate in the WHS meeting.
Meet with the WHS committee (the Chairperson and at least one other committee member) to discuss a strategy to implement the new protocols.
You have been nominated as the note taker for the meeting. Recording the actions and decisions is important. You may write notes for the minutes during the meeting, but they must be typed up formally for submission to your assessor after the meeting. Work into the WHS meeting minutes template.
The meeting minutes should detail specifically what your contribution to the development and implementation for this protocol is.
During the interaction, a number of tasks will be allocated to the committee members with due dates. You are required to actively participate in the meeting by offering your ideas and opinions, raising concerns and nominating yourself for a task.
In Step 5 of this assessment, you will develop a procedure and a strategy for implementing one aspect of the new protocols.

Complete your contribution to the new infection control protocols
In this step, you will develop a procedure for implementing one aspect of the protocols that you agreed to in the WHS meeting. You might be responsible for implementing:
limiting of visitors
wearing of masks
improved hand washing
reducing the amount of time that residents spend outside of their rooms.
Research the effectiveness of your chosen area by reading and interpreting existing WHS policies and procedures for your simulated workplace and by reading updated information about controlling infectious diseases in aged care on Australian government health websites.
Develop a one-page procedure to support the implementation of your new protocol. The way that you present your procedure is up to you it might be a formal, one page written procedure, a poster, an infographic or you might have another idea.
During the development process, email a draft to your supervisor (your assessor) and request their feedback.
Incorporate your supervisors feedback and check the document for neatness and accuracy before finalising your procedure.
Send an email to your supervisor (your assessor) with the final copy of your procedure. Include a brief summary of how you will implement the procedure in your workplace. This may be by sending an email to all staff and talking to residents and their families. It might be by putting posters up around the building or you might have another idea. Request formal approval from your supervisor to implement your procedure.
Implement the new protocol
Based on the agreed action in the WHS meeting and final approval in writing to implement, you should go ahead and implement. The way that you do this will depend on your proposal and the agreements with your assessor. Some examples include:
Emailing your procedure to the people who were at the WHS meeting (if your procedure is for staff)
Putting posters up in your RTOs simulated workplace
Emailing your assessor and a fellow student who will role play family members.
Note: You will need to provide evidence of having implemented your strategy. Talk to your assessor about the best way to do this. If your implementation strategy involves email, then copies of the emails will suffice. Other forms of suitable evidence include photographs of your posters on display or a video of conversations with residents or family members.

A position of supervisor has become available at Sheep Stations Nursing home where Mary, 62 and Andrea, aged 22, work. Mary has worked at Sheep Stations for 15 years and is a highly respected member of staff. She was a registered nurse in her early career and has been a personal care worker for the last 20 years. Andrea has been working at Sheep Stations for 8 months and is a qualified personal care worker as well. She is also well liked by staff and residents.
Both women were encouraged to apply for the position of supervisor by their respective managers. Both applied and neither one was successful. Human Resources told them both that they were unsuitable and the position was advertised externally.
Mary and Andrea asked for feedback from HR so they could improve their skills. HR did not respond to their requests but a panellist informed Mary that management said she was too old, and she would most likely retire soon. The same panellist informed Andrea that she was too young for a supervisor position and that staff would not respect her in that role.
Hayden has just been employed by a National Company to work in one of their facilities. Hayden was interviewed at head office and was impressed with the companys ethos and reputation.
Upon arriving to work on his first day at his new workplace, he discovers that the workplace is not accessible or set up for a person who uses a wheelchair, which he does. Robyn, the facility manager, tells Hayden they have never had anyone in a wheelchair because the building is old and does not meet current Australian Standards.
After several phone calls from Robyn, requests and meetings, head office refuse to make any adjustments to the building or make provisions for Hayden. He has no choice but to quit his job.
Rosewall Facilities have an indoor pool and gym and have set aside two hours on two days of the week where the pool and gym are only open for female staff and clients to use. This timetable has come about due to the high number of requests from female staff and clients, which also includes an increasing number of Muslim women.
There have been complaints from several men (both staff and clients), suggesting that they should be allowed to use the facilities at all times, that they are being discriminated against and that Rosewall are breaking the law. They were also not given any notice of the changes.

The town in which you work has recently seen an increase in migrants and your workplace has employed five people from one community. These five people speak little to no English, cannot write in English yet and have not been employed in Australia before.
Management have told supervisors to do whatever is necessary to ensure that the new employees are inducted and trained appropriately and that their integration into the workplace is smooth.
The Supervisors are angry that five non-English speaking people have been employed. They are not equipped to deal with this situation nor do they have time so do nothing except put them to work. The new employees receive no formal training, do not understand OHS requirements and cant fulfill the work requirements. The supervisor constantly complains and calls them stupid. One staff member slips over on a wet floor that she has just mopped. The supervisor is heard yelling at the employee Dont you know anything? Cant you read the sign? Youre an idiot!

Successful completion of this assessment task requires you to demonstrate your skills and knowledge by working respectfully with people from diverse social and cultural groups and situations, including Aboriginal and/or Torres Strait Islander people and identifying ways to work on your own perspectives and knowledge. In this portfolio you will need to demonstrate that you have worked with people from diverse social and cultural backgrounds in three different situations. The three situations should be with three different people. It is important that you provide evidence that you have completed this requirement and we have provided you with a Portfolio Template to work into.
You will collect most of your evidence and complete portfolio documentation during the work placement phase of your course. You may also complete some of your portfolio in the classroom. The rest can be completed in the classroom after your work placement is finished.
Your assessor will visit you in the workplace to observe certain aspects of this assessment and your workplace supervisor will also provide feedback.
Before you begin this task, meet with your workplace supervisor to discuss the people who will be most suitable for you to work with to complete this assessment task. Keep in mind that the privacy and confidentiality of the people that you work with are essential. There will be requirements around informed consent from the people and, in some cases their family and/or significant others and your work placement organisation. Your workplace supervisor and assessor will guide you in relation to your informed consent requirements.
The skills and knowledge that you are required to demonstrate in relation to working respectfully with people from diverse social and cultural groups and situations are all included in your portfolio so look at this in detail before you begin your work placement.
In addition to identifying the three situations or people to work with, you will require access to all the necessary facilities, equipment and resources to complete this task. You may also require access to your workplaces policies and procedures.
Remember, it is important to keep the privacy and confidentiality of the individuals you work with as high priority. Therefore, you must deidentify the people who you work with according to your organisations policies and procedures. If you are unsure about how to do this, speak to your assessor or workplace supervisor.

There are four sections of your portfolio that need to be completed:
Section 1 Reflection
In Section 1 you will reflect on and identify your own perspectives and biases around diversity. You will also need to provide evidence of the action you have taken to improve your awareness and competence. You are required to fill in answers to all of the questions in Section 1 and have your workplace supervisor endorse your evidence.
You must complete a Preparation section of the portfolio for each of the situations or people you work with.
Section 2 Recognise and respect the needs of people from diverse social and cultural backgrounds
In Section 2 of the Portfolio you will describe the strategies that you used to work and communicate with in three situations. You are required to answer every question in Section 2 and have your workplace supervisor endorse your evidence.
Section 3 Supervisor report
In Section 3, your workplace supervisor will complete a report summarising your performance while working with people from diverse social and cultural backgrounds. Ask them to complete the report only after you have completed Sections 1 and 2.
Important note: If you have permission to make copies of the following documents, include these as part of your evidence by attaching them at the end of the portfolio. If you are not allowed to remove any workplace documentation from your workplace, then your assessor will be required to look at these documents when they visit you in the workplace.
You must make sure you receive permission to include any documents from your workplace and all private information must be redacted:
relevant sections of each persons individual support plan
workplace policies and procedures related to diversity.

Section 4 Assessor observation
In Section 4, your assessor will visit you in the workplace and complete this section of your portfolio. Take a look at what the assessor will be looking for prior to the observation.
Submit your completed portfolio
Once you have completed all parts of your portfolio, had your workplace visit from your assessor and had your supervisor fill out the relevant sections required of them in the portfolio, you are ready to submit it to your assessor. Ensure that all sections are completed neatly and clearly, that all supporting evidence is attached and that it has been deidentified to protect the privacy of the people you worked with.
