1. Title of proposed study
2. Aims and objectives
What is the research question your study aims to answer and what objectives have you established to be able to achieve that answer?
3. Rationale
Include a brief, but appropriately referenced, synopsis of the background to the research (500 words maximum), leading to a stated purpose of the intended study.
4. Study Arrangements
Include details of the intended participants/data sources (include a sample size calculation where relevant), details of how they will be recruited, and (if appropriate) a copy in the Appendices of any advertising/poster material), where the study will take place, whether permission will be required for access to participants or data from an external source(s), the design of the study identifying the key variables/performance indicators (dependent & independent), inclusion/exclusion criteria and any controls/restrictions to be imposed on the participants, the data collection/processing procedures, and the intended statistical analyses. Additional information such as a schematic/figure of testing (i.e. design and conditions or, laboratory set up), detailed measurement protocols, or evidence of analyst reliability, can be added to the Appendices. 
5. Ethical issues
What are the ethical issues associated with your research, including the possibility of causing harm or distress to the research participants, the researchers involved, and the environment? Indicate how the ethical issues outlined above going to be managed/mitigated against, whether there are any potential conflicts of interest, what the potential benefits of the research are? Where appropriate, include (as appendices) a health screening form and completed ‘risk assessment’ form.
6. Informed consent
Indicate if informed consent will be obtained from research participants. If so, include a copy of the informed consent form as an appendix and give details of who will obtain consent and how this will be done, including how long participants will have to decide whether or not to take part. If not, explain why informed consent is not required. State what will happen to partially collected/incomplete data. If participants under 18 years are to be recruited, indicate whether consent will also be obtained from parents/guardians.
7. Participation information sheet
Provide this as an appendix (where applicable), written in uncomplicated language on two sides of A4 maximum.
8. Other arrangements
Indicate that you will comply with the requirements of the UK Data Protection Act (DPA) 1998 with regard to computer storage of data (securely for at least 10 years) and processing of participants’ personal information, and that generally the data supplied and generated during the course of the study will remain confidential.
