In this assessment you will download and complete the necessary documentation from AHPRA to prepare you for application for notation on your midwifery registration for endorsement for scheduled medicines. Your application must be complete with all relevant questions answered (except payment details) and evidentiary documents included.
The form MUST be printed and completed by hand.
Please be aware that it can take some time to get the required supporting documents, and some need to be certified. It is recommended that you look at this assessment and start making the necessary arrangements early in the course.
You will then need to scan your application form and any supporting documentation and save into ONE PDF document.
This PDF document will be uploaded through the submission portal in the Assessment 3 folder on Blackboard.
