Health promotion plan outline
Introduction
Provide a short introduction which gives the reader an overview of the whole assignment (Introduction should not longer than half a page).
Background (30%)
Provide information of the target community, their health issue, and their needs. Discuss key determinants of health for the particular disease in the chosen community (such as age, gender, culture, lifestyle, socioeconomic status), the burden of disease (e.g. prevalence, incidence, mortality of the disease, hospitalisation, and healthcare cost for that disease. Explain its impacts on health outcomes of the chosen community.
Critical explain the importance and significance of your health intervention program to address health issues and community needs in the chosen community.
Health intervention program (25%)
Propose the health intervention by applying theory and conceptual framework of health promotion (which approach(es) that you will apply to your intervention -Medical, Behavioural or socioecological approach). Critically explain the consideration on appropriate level of health intervention (primary, secondary or tertiary level). Your intervention may involve 1-2 levels.
The plan outline (35%)
Apply the Ottawa Charter strategies of health promotion that is relevant to your intervention (Enable, Mediate, or Advocate). Please choose only the relevant strategies. Use health promotion planning frameworks (Module 4) to help outline your plan in the following headings:
Aim(s)
Provide aim(s) of your health promotion intervention. You can have 1-2 aims.
Objectives
Outline the specific objectives for your health promotion program. Objectives are the actions you plan to do to help you reach the aim(s). (2-3 objectives is sufficient)
Timeframe
Explain a planned timeframe for your health promotion program with important milestones, including needs assessment, program planning, program design, implementation, evaluation, and reporting program outcomes.
Stakeholders
Consider relevant stakeholders that should be involved in your program such as health professionals, local government, Not-for-Profit organisations, schools or other groups.
