Case Study
Background information for Jodie
Jodie is a 17-year-old young person. She was born full-term, nil problems were observed at birth. Jodies mother stated she experienced post-natal depression after Jodies birth, and was briefly hospitalised in a mother-baby psychiatric unit. Jodies mother stated this was because she had a child two years older than Jodie, who died from SIDS just before she became pregnant with Jodie. Jodie is an only child.
Jodie described her childhood as normal, I guess. When ask how she would describe her parents, Jodie stated that her dad was an engineer who spent a lot of time at work, and her mother worked in human resources at a major bank. Jodie described living in a big house in a nice, leafy suburb. She stated her mother spent most Sundays doing meal prep for the week, carefully noting the nutritional content of the food that they would all eat. Jodie stated, mum likes us to be healthy, but she wondered whether this was because Jodie was overweight as a child, and her mother was embarrassed to be seen with her. In general, Jodie stated that as a child, she was expected to behave, play quietly, and do well in her music lessons and schoolwork. Jodie stated her parents were happiest when she excelled at school and received top marks in her classes. Jodie stated that at these times, both her mother and dad spent a bit more time with her, and they would celebrate by having a special treat, such as an ice-cream.
Jodie described herself as a worrier like mum and used to have nightmares that she would fail tests in primary school. She stated her dad liked to listen to her play the piano and cello, and that he entered her into several talent shows. Jodie stated she hated this and became physically sick once just before a show when she was 8 years old and could not perform. Jodie stated that since that time, her dad had not pressured her to perform, and she felt like she let him down. She reported that she would often stay awake at night wishing she could perform and would often become teary when she thought about it.
Jodie stated that one year ago, she studied so hard on her exams over a week that she forgot to eat regularly. Jodie stated that both her parents noticed she had lost weight and praised her for taking control of her eating. Jodies friends at school also noticed, and reportedly told Jodie that she looked great. Jodie was pleased, as she had always been ashamed of her body, particularly since puberty.
Four months ago, Jodie had lost so much weight that her parents were very concerned and booked her in to see a psychologist. Jodie continued to restrict her intake, became dangerously underweight, and she was admitted into hospital and an eating disorder clinic for three weeks. Jodie was discharged from hospital one month ago. Since this time, Jodie has continued to ruminate that she has let her parents down, and that because of hospital, she is even more behind in her schoolwork. Jodie stated she felt pressured by school and her parents to do well. She was supposed to apply to Sixth Form (advanced studies), but she felt sick at the thought. She has trouble sleeping at night as she stated she does not know how she can get all her work done.
Jodie stated that her friends did not understand why she did not eat, but they continued to have contact with her though her hospital stay, which Jodie felt grateful for. They have also organised a few catchups since she has been discharged.
Jodie is responding well to her current psychologist, and her blood tests reveal she is healthy. Her parents also want to attend family therapy so they could work through Jodies concerns together.

Using the information in the interview shown in the above video clip, as well as the background history provided in this assessment material, you are required to write a psychological report about Jodie. The report should contain below subheadings. The report MUST include:
a. Reason for referral (55 words). Reason for referral/client session
b. Mental State Examination -MSE (550 words). Use the following as headings in the report, and comment on each aspect, even if there is no disturbance present:
a. Appearance and behaviour (physical appearance, attitude, activity level/psychomotor behaviour)

d. Formulation (550 words). Using the background history provided, as well as the information in the interview clip, complete the 4 Ps for the biopsychosocial model of case formulation. This can be done in table format (see below for an example), using dot points.

Using the information in the table above, write a short summary statement focusing on the key factors that led to the presentation of Jodie. Describe how these factors interact to lead to the current situation and level of functioning, prognosis, and potential areas for intervention.
Question 2: Further psychological testing
Using the information about Jodie from Question 1, consider psychological tests that could be done to obtain further understanding of her current symptoms (e.g., severity), and the impact it has on her life.
In this section, you will write 880 words on TWO psychological tests (i.e., 440 words per test) that you think would be beneficial to administer to Jodie. The tests can be self-report, performance, or informant tests. You MUST use references to support your decision to use the two tests. Throughout this section, your writing should be in your own words and written in a formal academic style. You should reference literature to support your statements and cite these appropriately in a reference list. It would be clearer if you describe one test in detail, followed by the second test, to avoid confusion.
a. Firstly, you should justify why you have chosen those tests for Jodie. What information will you get from the tests to further your understanding of her condition? For example, you may decide to use a test that will reveal if Jodie meets the clinical cut-off for a disorder, or a diagnostic test.
b. Describe the psychometric properties of the tests you would give Jodie. Are the tests both reliable and valid for Jodies demographic (e.g., age, culture)? If applicable (and available to you), what are the test norms and clinical cut-offs? You should highlight important psychometric concepts that are relevant to your chosen tests (e.g., validity, test-retest reliability). When you refer to these psychometric concepts, you should define them, and use citations as appropriate. Comment on the method of administering each test, including the time it will take Jodie to complete each test.
c. What are some of the advantages and limitations of each test? Support your statements using research from the psychological peer-reviewed literature.
You must also demonstrate that you have considered the Australian Psychological Society ethical guidelines of psychological testing in your responses (across questions 2a, 2b and 2c). For example, showing the tests you have chosen are relevant to Jodie, and that she would benefit from gathering further information would be evidence of the ethical guideline: B.13.3 Psychologists ensure that they choose, administer and interpret assessment procedures appropriately and accurately.
