Unit Learning Outcomes
You will demonstrate the following Unit Learning Outcomes on the successful completion of this task:
ULO1: Describe partnership models of care for nursing with particular reference to working with vulnerable populations
ULO2: Appraise the impact primary health care principles have on health outcomes locally, nationally, and internationally
ULO3:Develop communication and cultural safety skills that contribute to impactful partnerships, particularly with vulnerable populations and those living in areas of rural and remote Australia
Task Description:For this assessment you are to imagine you are the Registered Nurse outlined in one of the four case studies and working in a community setting with the target population described. You have been approached to develop a community Health Promotion Project which you will present (via video) in Assessment 2. In Assessment 1, you are setting the scene for Assessment 2. For your project to be successful you will need to discuss partnerships with consumers, intersectoral collaboration and relationship building.
Rationale:
The purpose of this assessment task is to give students a greater appreciation of the principles of partnership, primary health care, and the health promotion work of a registered nurse. It also provides students an opportunity to apply their knowledge from the first 4 modules of this unit which focus on Partnerships, Primary Health Care, Health Promotion and Global considerations. Using a realistic case study they must think critically and practically to prioritise care, identify issues and promote the health and wellbeing of the community.
The assessment gives students the ability to apply knowledge of primary health care and health promotion to a practical and resourceful project while demonstrating engagement with an understanding of core content delivered across the first four modules.
Recognising how health care is prioritised in community settings will give students a broader understanding of the health issues they may encounter in all settings.
Task Instruction:
Step 1:
Read through the four Health Promotion Case studies and choose one of them to write about in this assessment. You are to write about the same case study throughout this assessment. The scenario examples written below are not to be used in your Assessment, you are to use one case study that has been provided to you. The case study you choose for Assessment 1 will follow through to Assessment 2, where you will develop the Health Promotion Project for the health issue in your target population.
Step 2:
Using the template provided, write your assessment using the below response structure:
Introduction to health issue:
Using an academic writing style, describe the health issue in your chosen case study and use scholarly references to support your claims. Discuss why this health issue is a concern in your target population and the outcomes for the community members if the issue is not addressed.
Example: if your case study was to increase bowel screening rates in 50-74-year-olds living in rural communities, research and discuss the consequences of low bowel screening rates in this target population, why this is an issue, why the rural community might be less likely to participate in bowel screening, and what the outcome will be if the target population are not screened and consequently bowel cancers are not identified early.
Approx. 250 words. Provide at least two scholarly references.
Principles of Partnerships:
Choose two Principles of Partnerships you will utilise when communicating with your target population and discuss why working together in partnerships is important for the success of your Health Promotion Project and how you will work with the target population and partners.
Example: if your case study was to increase bowel screening rates in 50-74-year-olds living in rural communities, and one of your chosen Principle of Partnership is Working Together, discuss why working together in partnerships with the rural community is important for the success of your Health Promotion Project and how you will work with the target population and partners.
Approx. 225 words. Provide at least two scholarly references. Hint Module 1 content.Primary Health Care Principles:
Choose two Primary Health Care principles, discuss why they were chosen in relation to the health issue in your case study. Example: if your case study was to increase bowel screening rates in 50-74-year-olds living in rural communities, and one of your chosen Primary Health Care Principles is Community Participation; discuss how this Primary Health Care Principle relates to your health issue and give examples of how you will use the Principle in your Health Promotion Project, to ensure its success.
Approx. 225 words. Provide at least two scholarly references. Hint Module 2 content.
Cultural Safety Principles:
Identify at least one Cultural Safety Principle you will need to address when communicating with the target population in your case study.
Example, if your case study was to increase bowel screening rates in 50-74-year-olds living in rural communities, and your chosen Cultural Safety Principle is Cultural Respect, discuss and give examples of how you will show Cultural Respect to the rural community, ensuring you do not diminish, demean, or disempower them through your actions.
Approx. 150 words. Provide at least one scholarly reference. Hint Module 2 content.
Sustainable Development Goals:
Choose one Sustainable Development Goal (SDG) that links to your case study and Health Promotion Project. Discuss why this SDG is important to your community and how your project will meet this goal.
Example, if your case study was to increase bowel screening rates in 50-74-year-olds living in rural communities, and your chosen SDG is Goal 4 Quality Education, discuss how Quality Education links to the case study and health issue, and why it would be important to your community and project.

Line spacing for your written assessment should be either 1.5 or 2 spacing.

Word count rules

At least eight quality peer-reviewed and evidence-based resources references would be minimally adequate for this assessment item.
The unit modules should not be used for citing references.
Direct text quotes should be used at a minimum or but preference is they are not used at all. Paraphrasing (written in students own words) is the key to demonstrating your understanding of the content.

Acceptable scholarly reference sources should be no older than 10 years unless they are seminal pieces or a key piece of literature that remains in use and remains relevant, for example:

Submission
The assessment should be submitted to Turnitin via Assessment 1: Foundations of Community Health Promotion Project in the Assessments Tasks & Submission section on the Blackboard NURS2018 site. Only Word documents submitted via the Turnitin portal on Blackboard will be accepted.
All late submissions without Special Consideration will incur a deduction of 5% per day of the total marks available.
Special Consideration requests need to be submitted on or before the due date through the Special Consideration Portal. Applications will be assessed on a case-by-case basis according to SCU policy. SCU special consideration
No more than two special consideration requests for this assessment task will be considered, unless extenuating circumstances apply as approved by the Pro Vice Chancellor (Academic Quality), or authorized nominee.
In the case of an emergency, please notify your Unit Assessor as soon as possible. Evidential documentation will be required to support your special consideration application.
