Theme of this reflection should follow the principle below:-Identify a patient who you have cared for in practice during year 2 of study.
This patient must have needed some partnership working/collaborative working with members of the multi-disciplinary /multiagency/interdisciplinary teams in order to meet their person-centred care needs. These could be- for example- social workers, drug misuse team, primary care teams, specialist teams; or 3rd sector/voluntary/charitable/emergency service professional.
Avoid basing your reflection wholly on teamwork with the nurses on shift in your own ward/practice as this does not fully reflect partnership working. You can of course discuss the nurses role within partnership working.
Your aim is to meet learning outcome 4:- Critically reflect and discuss how working in partnership with other professionals might enhance the impact of care across a range of diverse settings.
NB:- You must provide a confidentiality statement aligning to professional codes of conduct- you can also develop this in relation to other legislation. If this is breached you will receive an AUTOMATIC FAIL- so do not state ward names/hospitals/staff names/ patient and family member names.

1. You must frame your assignment using Gibbs cycle of reflection- you must discuss how Gibbs will facilitate your reflection. Refer to professional codes of conduct and/or other legislation to aid this discussion. You must provide Confidentiality statement. Should be written at level 5 ability.
2. Gibbs cycle:- Description- stage 1- introduce your patient with brief description of their health and social care needs i.e. reason for admission/being on caseload, PMH; and the key professionals/other agencies who were involved in delivering care to this patient. Please do not list these roles. Approx. 250 words for above intro and stage 1
3. Feelings and thoughts stage 2 - with reference to person centred care key documents, how did you feel the patients needs and wishes were taken in to account and why do you think this?- for example- does this follow the principles of PCC or shared decision making, or was leadership a barrier to IPW and patient care?- provide discussion. Were there any thoughts from other members of the MDT/IDT/MAT or family/carers involved in the care, that you could critically reflect upon? Approx.300 words
4. Evaluation- stage 3- Define and discuss the overarching principles and/or theories of partnership working, and then critically evaluate this against what happened, and what you and others contributed to the situation i.e. apply this theoretical discussion to what happened in your practice. Approx. 300 words
5. Analysis- stage 4- Critically reflect and analyse upon the key barriers and enablers to partnership working in relation to your identified patient and why the barriers/enablers occurred in the delivery of care. Link to your initial reflective trigger as stated in feelings and thoughts and evaluation. You can link to relevant policies/drivers/legislation/NICE/models/frameworks that promote partnership working in your setting. Any of the content discussed during the module may be relevant to your reflection and can be used to develop discussion. NB- you do not need to refer to all content from the module- only those themes that are relevant to your trigger- this will allow you to really go deep into the literature and create critical analysis Approx.900 words6. Conclusion and Action Plan stages 5 and 6- discuss what you have learnt from this event and the impact that working in partnerships with other professional and/or agencies had on meeting the person centred needs of your patient i.e. what will you take forward in to your future practices. You must discuss your action plan in consideration of your own knowledge and skill development in being able to contribute to partnership working; in addition identify how you will develop your skills.
