This assessment task requires you to refer to the case study provided
and address each of the following points from the clinical reasoning cycle:
Describe consider the patient situation (approx. 100 words).

primary/secondary survey framework to undertake this activity. Identify and provide
an overview of the pathophysiology of the main health challenge evident in the case

expected findings and identify information that is not relevant to the scenario (approx.
200 words).
Synthesise making sense of the data, understanding the clinical manifestations
(approx. 300 words).
Establish goals -identify one main goal for the outcome for this patient using the
SMART goal framework (approx. 300 words).
Select a course of action outline 1 nursing interventions that will assist the patient
to meet the desired outcomes and provide basic evidence-based rationales for each.
The intervention should be selected based on priority and urgency ( approx. 200
words).
Evaluate explain the process for evaluating the desired outcomes in this scenario
(approx. 200 words).
Contemplate reflect on process and new learning (approx 100 words).
