Ola Williams was born on 21/6/78 and is currently admitted to hospital after a fall in the street, where he was found intoxicated with a fractured radius. He was brought in by ambulance into ED and has since been transferred to an orthopaedic ward as he requires surgery on his fractured arm.
Ola lives in a hostel; he has one room and shares a bathroom. There is a support worker who attends the property regularly. Ola’s room does not have many personnel possessions. He has been living there for 3 years, before that he was homeless. Ola reports that he does not have any family. Ola grew up in care, he had several foster carers throughout his childhood. He was removed from his family when he was 5, due to concerns around domestic abuse at home and neglect. His mother has since died due to alcohol related disease. Ola previously used crack cocaine when he was homeless, he reports that he has been clean after engaging with Addaction. He has now missed several appointments.
Ola does not have any problems with verbal communication, but when he was younger, he was diagnosed with a mild learning need and ADHD, so can have difficulty taking in and retaining information. Ola does not have any breathing difficulties, he does smoke cannabis when he can, when he has enough money.
Ola reports that he can miss meals, even though the hostel support worker ensures that the residents do have food available. Ola has used alcohol for many years, varying in severity at times. Ola does not have any problems with elimination and has access to shower facilities at the hostel. He states that he showers once a week. On admission his clothes appeared worn, and his shoes had holes in the sole.
Ola does not have any problems controlling his body temperature. He does not need any walking aids and walks independently; he does have a limp. When he was homeless, he was assaulted by a group of people and fractured his leg. After treatment he found it difficult to attend his fracture clinic appointment and developed an infection.
Ola has had previous mental health problems and was admitted to hospital a few years ago with a psychotic episode when he was homeless and using crack cocaine. Since his discharge, he has remained resident at the hostel. Ola was employed several years ago as a gardener for the council but hasn’t worked for a while. Ola had used to enjoy playing cards with his friends Trevor and Colin. They used to live at the hostel but have since moved on.
Ola has a girlfriend called Ashleigh, he met her at the Addaction group, he has not seen her for a few days. Ashleigh is currently homeless, and her mental health has declined. Ola has needed medication to help him sleep, which has been prescribed this through his GP. His GP has been supportive in the past referring him to counselling and Addaction. Ola has not seen his GP for several weeks now. When his mental health declined, he did have thoughts of suicide. He has not expressed any thoughts of this in recent years.
