Purpose This assessment is assessing Learning Outcome 3 which requires you to critically analyse the interaction between communities and their social and physical environment and the ways in which these factors may impact on a persons clinical presentation, access to health care and ability to comply with a health plan within rural and remote areas.

Description This short essay requires you to choose a priority health condition and explore person centred models of care that would be suitable in promoting health and wellbeing including risk factor modifications for your chosen health condition.

This assessment is assessing Learning Outcome 3 which requires you to critically analyse the interaction between communities and their social and physical environment and the ways in which these factors may impact on a persons clinical presentation, access to health care and ability to comply with a health plan within rural and remote areas, so please ensure you address these concepts within your essay.

NOTE: Short essays can be more difficult to write than longer assessments so you must be succinct and choose your concepts for exploration carefully. You should only include a very brief introduction and conclusion.
