Part 1: Identifying the importance of evidence-based practice to nursing practice
In part one, you must explain why evidence-based practice is important in nursing practice. As part of your answer, you must identify the link between evidence-based practice and the NMBA Registered Nursing Standards. Your answer should be supported by at least tworeferences. These two references do not include the NMBA Registered Nursing Standards.

Choose oneof the following scenarios to answer parts 2, 3, and 4.
Scenario one: Alex Thorne You have recently begun work as a registered nurse in a small private hospital. You have been handed over the below patient, Alex Thorn Alex Thorn is an 87-year-old gentleman who has been living in a residential care facility for the last five years. He is demonstrating signs and symptoms of delirium due to a urinary tract infection. He is feeling hot and restless, and also feels an urgent need to go to the toilet to urinate, frequently. He recognises his surroundings, but does not know where he is. He wants to see Heather, his daughter. You are familiar with the symptoms of SEPSIS and ask the nurse handing over to initiate the Hospital SEPSIS pathway. The nurse indicates that she is unaware of a SEPSIS pathway and states Yeah, we should keep an eye on it, but for now lets just get him some additional analgesia and hold off calling for an escalation in care, as I dont really want to bother the consultant. I heard that they are on a very busy list today

Scenario two: Stan Checkkets
You have recently started working as a new graduate nurse in a medical ward of a private hospital. You are receiving bedside handover for the 2pm afternoon shift from the morning nurse about Stan Checkkets, a 72 year old who was transferred to the ward from the Emergency Department where he presented with complaints of abdominal pain and vomiting over the past few days. Stan presented complaining of feelings of being bloated, dizzy, and weak. Stan has a past medical history of cholecystectomy, appendicectomy, right inguinal hernia repair and takes Atorvastatin and Ramipril. When you walk into the room Stan is lying in bed. He looks pale and his abdomen is visibly distended. He is moving frequently, grimacing. Stan has IV fluids running and 2L of oxygen via nasal prongs. Stans last observations were taken at 10:00am. HR 110 bpm BP- 125/75 RESP- 20 talking sentences O2 sats > 96% with oxygen Neuro- GCS=15 Temp- 37.4 degrees Pain 6/10 cramping intermittent cramping You notice that a GIT assessment has not been conducted since arrival on the ward and bowel sound assessment has not been recorded. The nurse informs you that during the handover from the emergency department, it was requested that a Nasogastric tube be inserted. However, this was not done overnight or in the morning. The nurse indicated that she did not think that it was urgent as the patient had passed some diarrhea in the morning. Stan complains of feeling unwell and has a large vomit.

Part 2: Identifying a clinical problem
In each scenario, there are concerns that staff are not following the most up-to-date evidence-based practice. You discuss your concerns with the Nurse Unit Manager who asks you to compile evidence to support your concerns and identify best practice.
Using ONE of the chosen scenarios you must:

Part 3: Developing an answerable clinical question: PICO
Develop a clear and answerable question relating to the clinical problem identified in question 2. Use the PICO framework to develop this question. You need to write your full question and then provide a breakdown of the PICO components.

Part 4: Database searching
You are required to demonstrate an advanced search strategy relating to the PICO clinical question you developed in section 3. Use the database CINAHL to do this advanced search and record your results. You must:

Search Strategy List the search strategy you used using advanced search techniques*. List the results for each search for one database (CINAHL).A minimum of fouradvanced search resultsare required. Your final search line should list the limiters that you applied to the search and the result when these were applied.
To answer this section, follow the Examples of literature searching using PICO in Module Two: Topic 2.3 Searching for the best evidence. This example demonstrates how to conduct and display your search. Use one line for each search term and its synonyms, then combine the search terms using the search number for each row (S1 and S2 and S3). This will provide a result and then you apply your limiters. Add your limiters as the last part of the search. You may need to adjust your search if you get zero or very low results. This technique is an important research strategy.
*Ensure that you demonstrate advanced search techniques: Boolean operators and at least 2 examplesof Phrase Searching or Truncation Symbols or the use of Wild cards.
