Assessment 2 Case Study Report: Older person care event (falling short of quality, safe care) and influences on practice **Make sure you use choose a different client to Assessment 1 for the focus of this Assessment Background Information Critically analysing older peoples care experiences within a broader social, cultural, historical and political context is useful in raising our conscious awareness of different influences that impact care that may be hidden from our view. This raised awareness about these aspects of care improves our understanding as nurses and assist us in directing action strategies that can improve care of older people and their family from a multi-leveled approach. Instructions: 1. Write a 2000-word report that critically analyses a Case Study within your clinical practice that you have identified as falling short of quality, safe care. This analysis is retrospective, to enable you to explore the various aspects and influences (historical, social, cultural factors and perspectives, challenges and issues), impacting the care practice of this older person. 2. The following information is provided to assist you in structure of the essay and to guide writing the critical analysis. Structure Report Title Page Table of Contents Introduction (One paragraph) Thesis statement. Introduce your older person (do not use real names). Orientate the reader to what you will be writing about (include main points). For example, you may like to introduce your client this way: A fragile 89-year-old Caucasian male, living in a private Residential Aged Care Facility (RACF) in Western suburbs of Sydney had a suspected Urinary Tract Infection (UTI). Etc...). Body (Eight paragraphs the following is a guide only) Include a brief history (as relevant to the case study and situation). Provide brief details about the care that was provided for that situation. Provide details about the rationale for that particular care practice for the client. Describe what happened during the clients care that you identified as being a concern and or problematic? Provide details of how you evaluated that care practice to form a judgment that the care fell short of quality, safe care Describe and critically analyse the historical, social, and cultural factors that influence this care practice? How did the care practice/situation, impact on the client, family, department, organisation and nursing profession? (What were the issues and challenges?) Who benefited and or was disadvantaged by the practice? Detail the power relations between these groups? Provide quality improvement strategies that can improve the care of your client and their family specifically, and generally for older people, from a multi-leveled approach (patient/individual level; departmental level; organisational level; & Governance level). Provide justification for these quality improvement strategies. Please ensure your written statements within your body paragraphs are supported by reference to relevant literature. Conclusion (One paragraph) Include a summary and link back to: Thesis statement Include overview of main points and synthesis of ideas by linking all concepts and elements together. Recommendations Include a set of recommendations (QI strategies) to improve care. These need to be multileveled inclusive of: (patient/individual/family; ward/unit//department; organisation; and environment levels. |
