Summary and Purpose of Assessment

• This is the third (3rd) of three (3) assessment tasks for this unit. This assessment task allows you to demonstrate the knowledge required to implement an individualized oral hygiene program
• This assessment task, is based on the knowledge required to recommend and implement a prescribed oral health education program, as demonstrated in Assessment task 1: Clinical logbook.

Assessment Instructions

You are required to:

Conduct a presentation showcasing ONE (1) patient case that you have managed at your clinic (chose one case from Assessment Task 1)

The presentation must include:

• a 15 - minute presentation on a clinical case using PowerPoint slides
• one patient form your logbook (i.e., identified by their patient number)
• provide the rationale and strategies for the prescribed oral health (OH) plan you implemented
• evidence of any medical conditions present and how this impacted the OH plan
• a reflection on what you would do differently and why or why not
• refer to the detailed instructions in the section below

Where

• The presentation will be conducted in the classroom, where you will present to your Assessor and peers. Refer to Canvas for location details.

How

• You will be assessed against the criteria as listed in the observation checklist in section B of this document
• To achieve a satisfactory result, you will need to address all the criteria satisfactorily.

You must submit a hard copy of the presentation slides covering responses to ALL questions listed in this document. Print off 2 slides per page, staple them together and submit them to the Assessor with the assessment task attached. You MUST submit this directly to the Assessor on the day of the assessment. Once the assessor has reviewed the documents then:

• Upload your assessment task in Canvas > Assignments > Assessment Task One.
• Submit a single file only, preferably in either Word or PDF format. If your submission contains files of multiple formats, then submit them all in a single zip file. Whatever file you submit, name the file as follows: your surname, your student number, unit national code, and task number:
• Example: Roberts_s123456_HLTDEN018_Task 3
