Improving Patient Care: Clinical Risk Action Plan
Assessment description
Complete a quality improvement action plan that addresses the risk/s identified via the
root cause analysis (RCA), in assessment 2.
This assessment will provide evidence towards the following unit learning outcomes:
- ULO 1 Examine the opportunities for nurses to create safe environments for
care using clinical governance principles - ULO 2 Critically analyse the relationships between clinical governance,
clinical management and health outcomes - ULO 3 Formulate a quality improvement plan to address areas of identified
clinical risk - ULO 4 Evaluate clinical leadership in nursing as a concept critical to patient
safety, interprofessional teamwork and transition to registered nurse - ULO 6 Reflect on working as a professional within a team culture to promote
a healthy working environment, including accountability, reliability, collegiality
and maintaining capability for practice

Assessment formatting guidelines
- Students will be expected to complete the assignment individually outside of
class hours. - Students must maintain full confidentiality in their case study by ensuring that all details are de-identified (including all names, dates, location and any other identifying factors).
- Students should use APA style 7th edition referencing format for their in-text citations and reference list.
- Students' names are not to be included on any assessment tasks/submissions. Only student ID numbers should be included (as per the Assessment Policy and Assessment Procedures
