Materials and Methods

Patient Recruitment, sample selection and Data Collection

The Study samples were selected from pregnant participants from the Queensland Family Cohort (QFC) based at Mater Hospital. 24 patients within 24-28 weeks of gestation are recruited for this study. The recruited patients were categorized into three groups, and this included :

Control group (uncomplicated pregnant patients)

GDM patient group treated with diet

GDM patient group treated with medication (metformin/insulin)

To minimise potential confounding factors, the study participants were matched as closely as possible for maternal age, body mass index (BMI), gestation at delivery and baby weight. Based on the QFC criteria, the inclusion criteria included the following:

Pregnant patients who are permanent residents of Australia and reside in QLD (39).

Participants who are 24-28 weeks pregnant (uncomplicated pregnancies, GDM patients treated diet and those treated with insulin or metformin medication).

Eligible individuals including those from non-English speaking backgrounds (39).

The exclusion criteria included the following:

Participant and partner consent was not given(39).

Women with severe systemic disease.

Women with preeclampsia

The ethical approval for this sub-study of the Queensland Family Cohort (QFC) was obtained from the Mater Research Ethics (HREC/16/MHS/113) (). The participants were approached for study participation by qualified experienced research midwives via SMS, phone or face-to-face. Furthermore, social media and marketing campaigns were used to recruit participants. Participants who were interested in the study and met the eligibility criteria were provided with sufficient information about the study before obtaining written consent.

Research Electronic Data Capture (REDCap) has been used to collect and manage data collected from participants in QFC (Mater Hospital) (). Questionnaires that included questions about lifestyle and health status are completed by eligible participants. Surveys completed by participants included demographic details (age, employment, education,), clinical characteristics, medical history, medication, and drug use().

Plasma blood samples were collected at 24-28 weeks of pregnancy and stored at -80C until miRNA extraction was performed.

miRNeasy/ plasma kit for purification of total RNA including miRNA and quantification using the Nanodrop

The miRNA was extracted from 500 L of blood plasma using the miRNeasy Serum/Plasma Kit (Qiagen, Hilden, Germany) following the manufacturers instructions (). Briefly, the kit included a QIAzol lysis reagent which is added to the plasma sample. Afterwards, chloroform is added to the mixture to prepare for the separation step. Centrifugation is done to separate the lysate into a colourless aqueous phase, white interphase, and organic phase. The aqueous phase is collected and then ethanol is added (). The sample is then placed on the RNeasy MinElute spin column, wherein the total RNA can attach to the membrane with the phenol and other contaminants being washed away (). In the final step, 14 l RNase-free water is added to elute the RNA (). The purification of cell-free total RNA will include the miRNA, which is then used for miRNA profiling . Afterwards, the Nanodrop spectrophotometer is used to assess the quantity and quality of the miRNAs obtained. The Nanodrop does this by measuring the RNA concentration in UV light .

Post-extraction enrichment

RNA samples (11 l) were diluted in 410 l of DNase/Rnase-free water (ZYMO Research, USA, Cat.No: W1001-4) and loaded into AMICON ULTRA 0.5ML 3Kda (MERG, Australia) columns and centrifuged at 14,000 g for 90 mins at room temperature. Columns were inverted and placed into new tubes and centrifuged at 8,000 g for 2 mins to elute approximately 25 l enriched miRNA.

NanoString Analysis for miRNA Profiling

The extracted miRNAs were measured using the NanoString nCounter system located at the Griffith University Central Facility for Genomics. The specific system used is the nCounter Human V3 miRNA Assay Panel which can rapidly and effectively profile 800 miRNAs in each sample (). The Human V3 miRNA Panel contains 799 probes that represent >95% of all human miRBase reads (). The assay provides direct, digital counts of each miRNA without the use of reverse transcription or amplification (). It is a form of targeted discovery and validation tool that allows the collection of expression data in a minimal amount of time with less hands-on manipulation (). Colour-coded probe pairs are used to establish direct molecular barcoding and digital detection of target molecules ().

The nCounter miRNA sample preparation method involves multiplexed ligation of the specific tags to their target miRNA and an enzymatic purification to remove unligated tags. The Tm-optimized bridging oligos that correspond to a portion of both the target miRNA and miRNA-specific tag as well as stepwise control of annealing and ligation temperatures are used to verify the sequence specificity of the ligation reaction (). The n-counter system has inbuilt positive and negative controls to improve performance ().

miRNA Sample Preparation Protocol

1 l of the miRNA Assay Controls was diluted in 499 l Dnase/Rnase-free water, mixed, and left on ice until use. 3 l of sample RNA was combined with 3.5 l of the Annealing Master Mix (13 l Annealing Buffer, 26 l nCounter miRNA Tag Reagent, and 6.5 l of the 1:500 diluted miRNA Assay Controls) in 0.2 ml 12-well strip tubes. The strip tubes were sealed with strip tube lids and placed onto a thermocycler running the Annealing Protocol (Table 2).

Following completion of the Annealing Protocol, 2.5 l of the ligation master mix (22.5 l PEG, 15 l Ligation Buffer) was added to each tube, mixed by flicking, centrifuged briefly, and returned to the thermocycler at 48C for 5 min. With the strip tubes remaining in place on the thermocycler, 1 l of Ligase was added to each tube, and the strip tubes capped. The thermocycler was closed, and the Ligation Protocol was immediately run (Table 2). Following completion of the Ligation Protocol, 1 l Ligation Clean-Up Enzyme was added to each tube, mixed by flicking, and briefly spun down. The tubes were returned to the thermocycler to run the Purification Protocol. Following completion of the Purification Protocol, 40 l of Dnase/Rnase-free water was added to each strip tube well and each sample transferred to sterile 1.5 mL microcentrifuge tubes for storage at -20C until use.

miRNA CodeSet Hybridization Setup Protocol

A thermocycler was pre-heated to 65C and 70C heated lid set to run infinite. Human V3 miRNA (241004) Reporter CodeSet (Lot. RC9901) and Capture ProbeSet (Lot. CP9789) reagents were removed from -80C storage and allowed to thaw on ice, protected from light. Samples were removed from -20C storage and allowed to thaw to room temperature on the bench. Samples were then denatured in a heat block at 85C for 5 mins and then transferred to ice to quick-cool.

In 0.2 mL 12-well strip tubes, 5 l of each sample was added to 20 l of hybridization master mix (130 l Hybridisation Buffer in the tube of Reporter CodeSet) per well, and 5 l Capture ProbeSet to each tube. Strip tubes were mixed by flicking, briefly spun down and placed in the thermocycler to incubate for 22 hours.

nCounter Sample Preparation

Nanostring nCounter cartridges and prep-plates were removed from cold-storage and allowed to thaw to room temperature for 15 minutes. A NanoString Max/Flex machine was programmed to run on high sensitivity. NanoString nCounter prep-plates were centrifuged at 2,000 g for 2 min at room temperature and loaded onto the Max/Flex Prep-Station. The nCounter cartridge was loaded into position and the 12-well strip tube with samples was removed from incubation and loaded onto the Prep-Station machine.

Following completion of the prep-station run, the cartridge was sealed to prevent evaporation of probes and transferred to an nCounter machine for enumeration.

nSolver analysis

The TECH NOTE nCounter miRNA Expression Analysis in Plasma and Serum Samples was used as a guide to normalise this data using a process that involves utilising the NormFinder algorithm in R, to find the most suitable genes within the data set to use as housekeepers for normalisation.

Raw data (RCC files) were imported into nSolver, and two experiments were generated (one for each run). The first step involved parsing out the most suitable candidate genes for processing using NormFinder. We used background subtraction of the mean of negative controls plus 1 standard deviation. We turned off all normalisation options at this stage and exported the normalised data to a csv file. There we excluded any endogenous or housekeeping genes with an mean expression less than 25 counts (RUN1) or 50 counts (RUN2).

We wrangled the data to fit the expected structure needed for NormFinder and ran the algorithm to return the top 5 genes to be used as housekeepers. Using this list, we returned to nSolver and manually included the 5 genes into the list of Housekeepers and discarded any preselected Housekeepers that had overly high coefficient of variation percentage (CV%).

Statistical analysis

Differential miRNA Expression Analysis

The raw data was normalized and analysed using the ROSALIND (https://rosalind.bio/), with a HyperScale architecture developed by ROSALIND, Inc. (San Diego, CA). Read Distribution violin plots, identity heatmaps, and sample MDS plots were generated as part of the QC step. The limma R library1 was used to calculate fold changes and p-values and perform optional covariate correction. Clustering of miRNA for the final heatmap of differentially expressed miRNA was done using the PAM (Partitioning Around Medoids) method using the fpc R library2. The top targeted gene predictions, validated genes, and related drugs and disease were analysed using the multiMiR R library3. miRNA secondary structures were calculated and visualized using the ViennaRNA software4.

Rosalind enabled Comparisons between the GDMD vs Control, GDMM vs Control and GDMD vs GDMM groups. The fold changes and p-values for comparisons were calculated using the t-test method (p 0.05). p-value adjustment was performed using the BenjaminiHochberg (p 0.01) method to estimate false discovery rates (FDR). For the purpose of this study, adjusted p-values were reported. MiRNAs with the fold change and p-adjusted value threshold of 1.5 and 0.05 respectively were considered significant. Fold change (1.5 for miRNAs upregulated and 1.5 for miRNAs downregulated), These comparisons provided an overview of the altered miRNA expression in the groups studied. The results were displayed in tables and visualised in violin plots and heat maps.

Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis using DIANA-mirPath v4.0

To supplement the analyses, the DIANA-mirPath v4.0 miRNA pathway analysis web server was used to perform the Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway) analysis using gene targets predicted by target scan v8.0. The KEGG database links genomic information with functional information for systemic analysis. Fishers exact t-test was performed to explore over-represented pathways in KEGG analysis, and results with p0.05 were considered significant. KEGG pathway was performed using miRNAs that were identified differentially expressed in the groups studied from the Rosalind analysis. This step is helpful in determining which biological pathways are represented more than expected within the set of genes or proteins of interest. This helps in identifying the key biological processes or pathways that may be involved in the GDM groups studied.

Results

The data was analysed by ROSALIND (https://rosalind.bio/). Based on the nanostring analysis, a total of 800 miRNAs were included in the Rosalind analysis. The limma R library1was used to calculate fold changes and p-values. miRNAs with a fold change greater than 1.5 and adjusted p-value 0.05 were considered significant.

Table 3 describes the log 2-fold change and p-adjusted values for the differentially expressed in the comparison groups. Whilst Figures 1,2 and 3 describe miRNAs that are upregulated and downregulated. When comparing GDMD and control, there were eleven significant miRNAs with a log2 fold change greater than 0.58 and p-value 0.05(). The significantly upregulated miRNAs in this group included hsa-miR-151A-3P,hsa-miR-3144-5P,hsa-miR-496,hsa-miR-520D-3P,hsa-miR-520H,hsa-miR-548AR-5P,hsa-miR-548J-5P and hsa-miR-570-3P(). The significantly downregulated miRNAs in this group were hsa-miR-3614-5P,hsa-miR-548V, and hsa-miR-627-3P(0.

Twelve miRNAs were significantly different between GDMM and the control group().The significant upregulated miRNAs included hsa-miR-219B-3P,hsa-miR-4455,hsa-miR-548G-3P,hsa-miR-548N,hsa-miR-561-5P,hsa-miR-627-5P and hsa-miR-548A-5P ().Hsa-miR-198, hsa-miR-603, hsa-miR-378H , hsa-miR-615-3P and hsa-miR-378G were significantly downregulated in this group

Only five miRNAs were significantly different between the GDMD and GDMM group. All of the miRNAs in this group were significantly upregulated and included hsa-miR-126-3P,has-miR-1322,hsa-miR-2117,hsa-miR-320C, and hsa-miR-502-5p ().

MiRNA target genes and KEGG pathway analysis

The differentially expressed miRNAs were selected for miRNA target prediction using the online tool DIANA-mirPath v4.0 with Targetscan v8.0., which is based on experimentally validated miRNAgene interactions. DIANA mirPAth v4.0 together with KEGG analysis was used to examine the significantly enriched (p<0.05) signalling pathways of the miRNA target genes and found that the differentially expressed miRNAs in all three groups were involved in insulin/glucose regulation.
