Part : Workplace risk assessment You are required to provide evidence of how you have conducted a workplace risk assessment and recorded the results while on your work placement. There are two components to this task: 
1. Using the template provided on the following pages, conduct a workplace hazard identification and risk assessment and identify and record at least one existing hazard and at least one potential hazard. 
2. Choose one existing hazard and one potential hazard that you identified in your hazard identification and risk assessment, and complete the risk assessment template provided, identifying: a) existing risk control measures b) current level of risk using the risk assessment matrix shown below c) at least one additional risk control measure using the hierarchy of control d) residual level of risk once the additional risk control measure has been applied e) who you reported this risk to, and when.

You must demonstrate your knowledge of WHS within a workplace environment by answering the following questions in the spaces provided based on your experiences while on work placement. Ensure you take note of the length of response requirement where indicated. In accordance with an appropriate role for a student on work placement, you are expected to follow guidance from colleagues and workplace supervisors in regards to accessing any workplace documents or technology to complete this part of the assessment. 
At the conclusion of this task there is a declaration that must be signed by both you and your workplace supervisor verifying the authenticity of this part of the assessment and your application of WHS while on work placement. 
1. Explain how you consistently followed the organisation's policies and procedures for safe work practices in the day-to-day work activities required by your job role, including: 
a) Infection control b) Hazardous manual tasks c) Use of PPE.
