6. Discuss and appraise current evidence for the LTC including appraisal of opportunities and challenges presented to nurses when assessing, planning and managing the 
LTC. Explain how the biopsychosocial elements may impact on this.

7. Explore the attitudes, values and beliefs which may impact upon self-monitoring and self-management of long term conditions.

8. Identify and explore aspects of attitudes, values and beliefs related to the LTC. This could include cultural, ethical, legal or safeguarding. This may include 
producing resources in other languages or consideration of where / how to use the resource.

9. Examine the nurse’s role in supporting, promoting and protecting the well-being of individuals living with a long term condition.

10.Consider how nurses will work collaboratively with individuals and members of the multi-disciplinary team to support, promote and protect wellbeing of the person with.

11. Develop a resource which promotes health and well-being based on collaborative principles

12. Identify a model(s) / framework(s) utilised in self-management related to the chosen LTC. Give a clear discussion of the possible ways of applying the model(s) / framework(s) to the selected topic and the resource created.

13. Design and develop a resource (for example web page, poster, academic game, workbook etc.) to increase awareness or which supports a long term condition (LTC)
