S2:Evidence-based practical report on planning health and social care provision.

This will be a more practical report, which will take into account the evidence coming from the Health and Social Care field related to role of professionals in the co-production of partnership with users of services. Your work with another colleague (peer- working on the parts below) will be very important towards providing some of the necessary evidence. Please document on a weekly basis the outcomes of the work with another colleague of yours in thereflective journal(roughly from week 10 to week 13) and use it throughout your writing for S2.

Your work for S2 can be structured as following:

-The contemporary role of the professional(around 800 words).

-Construct and deconstruct identities and roles related to HSC work, both of professionals and users of services;

-Describe contemporary approaches to personalisation and co-production;

-An understanding of users of services(around 800 words).

Use the concept of perceptual positioning to provide your peer-informed understanding of users of services. Please chose and describe one planning tool and technique that was used by your colleague to provide a person-centric service.

* TheEvidence-based practical report(S2) will be marked according to the following criteria; please also consult the marking criteria from Table below:

-Ability to show how identities and roles related to HSC work are constructed and can also be deconstructed (25%);

-Ability to explain key theories around key approaches to personalisation and co-production (25%);

-Ability to describe a planning tool and technique used by peers to provide a person centric service. (25%;

-Amount of research and range of sources used the amount of sources as well as their quality and justification of choice (15%);

-Written communication skills: structure (including word count), appearance, spelling, punctuation, grammar and paragraphing (10%).
