Summary of Session Content:

Kai mentioned in our 2nd session she is really confused how her new contract she gets is on the same day she works not a day earlier. she is having alot of clashes at work regarding her hours she is not getting paid for. She said she only gets paid for 12 hours whereas other 6 hours she doesn't get paid and she will be talking unfair work. She feels like there were some miscommunications with the manager but she feels like she communicates alot but feels maybe manager is not listening and then she gets blamed for.

Kai's childhood memories are so fuzzy she doesn't remember much but all she remembers is that she always had to listen to parents no matter what. she said decisions were controlled by her parents and with them it feels like bury my emotions and move on. She opened up that how rational person she is and she really doesn't express her emotions. She view herself as a failure and unsucessful person, she always used to have stomach aches when she had exams at school just like she has stomach aches when she goes to work.

She is avoiding friends at this stage as she feels like it takes alot of here time as they want to spend the whole day instead of couple of hours. she said she would like to communicate more with her friends but says it takes alot of her time as she likes to home most of the days when she is not working.

Homework Assigned: Rain technique worksheet to explore mindfulness techniques in anger moments.

Plan: Next session focus on case formulation like 5P's and she mentioned she really wants to focus on schema therapy and explore her goals.

Summary of Session Content:

Kai discussed challenges at work that she has been going through for last 2 years and at times that is making her frustrated at work and her anger self comes out.

kai talked about the struggles she is having at work. how she feels like she is always the one to blame even though she is willing to help the manager but she feels like she is never listened to. I explored some questions with her regarding her family relationships and friendship. she mentioned her relationship with family is not the best she rated 4 out of 10 and 10 being very good. she had one friend in past but she doesn't talk to her anymore often.

She talked about her relationship with boyfriend is not the best as she feels like she is not heard or supportive at times. She is unsure of her goals what she wnats to do as she sometimes believe like she is a failure.

She has spoken alot about work issues like how she was treated unfairly at work and how it has impacted her. she said she feels hopeless and frustrated.

She really wanted to focus on how to cope with her anger and emotions and she feels lost with things and lonely.

She doesn't like to open up more these days thinking what if she gets hurt and what if she doesn't expect what she was expecting from them as a response.

Homework Assigned: YSQ and core beliefs worksheet

Plan: Next session explore more of her feelings and what does she do in the moment she is feeling angry. Next session 20th June 2024 10AM perth time.

- Session #7 Zoom (2-hour session to make up for a missed session)

Session Agenda:

- Emotion Log (see attached)

- Case Formulation - Predisposing (see attached)

Measures Administered and Results:

YSQ: elevated scores across 3 sub scales. The three highest scoring were Self-sacrifice (4.67), fear of losing control (4.75) and unrelenting standards (4.86). Additional non elevated subscales to note are failure (3.50) and emotional deprivation (3.40). Additionally social isolation increased to 3.20 from 2.40 from when she previously filled it out.

Client Functioning and Progress (reported/observed):

MSE conducted see attached. No concerns observed.

- Mood rating: 6/10 (10 being comfortable with myself and the people around me) due to her feeling more in control of her life right now. Such as organising her trip overseas next year.

Risk Status:

no risk identified

Homework Outcomes:

- Completed emotion log (see attached) - Client reported feeling impassioned when she was with Alex's cousins and saw that they were speaking positively about him (e.g., he's so helpful, he tries). This made her feel impassioned because she did not believe it to be true. Client reveals that's she does not tell people about her concerns with her relationship as she usually gets told to "break up with him". It was discussed the pros and cons of staying with him. Same reasons mentioned last week.

Summary of Session Content:

- Emotion log (see attached). Client additionally reported questioning her sexuality, whether she may be asexual and is afraid to explore it. Client also reported her friendship with Lily being one sided and she feels an obligation to care for her (e.g., when she is suicidal). She describes her closest relationships to be with her volunteering friends who she has known for 5-years.

- To conclude the emotion logging discussion, client reported that while she does see a benefit to it (she is becoming more aware) she does not see the point in doing it for the overall goal. The therapist did psychoeducation on that when we become more aware of our emotions, that informs our behaviour. Client started to understand but was still hesitant as expressed by her saying "I guess".

- Case formulation (see attached) - client engaged responded well to discussing her parents as a predisposing factor.

Homework Assigned:

- continue emotion log

- Client to reflect on what she has learnt over the 8 sessions.

- Session #6 Zoom

Session Agenda:

- reviewing emotion log

- Discussing life satisfaction and relationship with partner

Measures Administered and Results:

YSQ: non elevated across all subscales. The three highest scoring were Sel-sacrifice (4), fear of losing control (4) and unrelenting standards (4). The subscales below that are failure (3)and emotional deprivation (3).

Client Functioning and Progress (reported/observed):

MSE conducted see attached. No concerns observed.

- mood rating, client put her baseline mood at a 3.5-4/10 (10 being positive). See homework outcomes emotion log for details.

Risk Status:

no risk identified

Homework Outcomes:

- Completed emotion log (see attached) - noticed that she typically only logs negative emotions. Client also noticed that she observed her positive emotions are not positive, but are instead an absence of negative emotions. She has felt at a 3.5/10 since 4 year ago when she started her PHD and got into a new relationship. More recently it has moved up to a 4. When asked why it has not changed as her PHD finished, client reported that she is not satisfied.

- YSQ - client self-scored and noted that she under-reported. Client will do the YSQ again for next time.

Summary of Session Content:

- The session was spent exploring client's thoughts and feelings about her relationship and how it has affected her. Client reports that due to her parter being complacent in his own life and in their relationship, she has also become complacent. Client reports thinking this might be because if she is too goal orientated it would put too much of a gap in where they both are in life. Client reports fearing that there might not be anyone out there for her if she breaks up with Alex (her partner). When asked, client reports she does not intend looking for a relationship. Client reports that there is also another person (Jojo) who has expressed his feelings for her. Client reports that she does not like him as he is too emotional (one time made pasta from scratched for her when she was craving it).

- it is observed that client may be avoidant of emotional attachment. It was also observed and discussed with the client that she feels uncomfortable with too much emotions is that she is pressured to try harder to be the better person, whereas with Alex, she does all the work.

- Client reports feeling envious of her other friends for experiencing the lives they want to live. Client reports not feeling proud of her relationship and being worried about what people think of their relationship.

- Client reports communicating all of her thoughts and feelings to her partner, and reports that he expresses wanting to change it, which he sometimes implements, but does not take it seriously and reverts back. For example, making goals because she told him to but not genuinely committing to the goals.

- Despite being unsatisfied in the relationship, client reports feeling secure as Alex (her partner) does not pressure her with sex, has already built a life with him (bought a house together, currently living with his family, on a partner visa), has worked hard on getting him to start doing what she needs (e.g., helping her with housework). When asked if she sees him ever fulfilling what she needs, client reports that he will ever reach 30%.

Client reported that she feels connected to her best friend that she grew up with in Singapore however they only speak a few times a year as their lives are busy. Client reports that her best friend understands her compared to anyone else - however they avoid topics of religion. Client additionally reported feelings that her relationship is more out of obligation, however it feels like the better part of her life right now.

- Client is observed to be protective of her relationship. Client reported being "envious" of her friend and her lifestyle however it was more about her friend being happy and exploring. Giving supporting evidence that client may not be ready to explore areas outside of her relationship.

- Goal setting - it was discussed that her goal is to identify where her problems may lie and explore it further to then carry on after termination for her to work on. Assessment Details and Instructions

Purpose:

The purpose of this task is to formulate and develop a case presentation. It is expected that students will complete this task by engaging with the unit textbook, authoritative sources, the Clinical Supervisor and peers during supervisory sessions and in the Moodle discussion forum. As well, students will incorporate your own professional reflections as part of your placement.

A case presentation analyses a real-life situation to develop counsellor understanding of client. It begins with a case conceptualisation based on a framework (with proposed assessments), implementation, interventions, analysis, recommendations and conclusions constructed over time, supported by relevant theories and staged feedback.

It is important to provide a balance in the class between the case presentation and students being able to talk about their placement experiences and concerns as this is an important aspect of supervision in terms of critical reflection on practice.

Assessment Submission:

There are ongoing components for successful completion and submission of this task:

Students will attend weekly Clinical Supervision sessions where real life and scenario client cases are presented and discussed. It is recommended that students

present at least 1 case to practice their case presentation and formulation skills to the group at times scheduled by the supervisor throughout the trimester before the final presentation

ORstudents present the same case twice and develop and improve that same case conceptualisation and formulation prior to the final presentation

(Please forward the written materials for the case presentation to your supervisor the night before you are scheduled to present).

2. Students will develop, refine and modify their case presentation skills over the trimester in conjunction with feedback from the clinical supervisor and peers.

3. Students will present theirfinalCase Presentation to the Clinical Supervision group in either Week 10 or Week 11 of the trimester. Suggested 15minutes for presentation and 10 minutes for questions or feedback.

4. By Week 12, Monday at 4 pm AEST/AEDT, students will submit the following;Students must ensure there are no identifying elements and they have ensured client confidentiality in their submission.

1 x PowerPoint presentation

1 x Word document consisting of:

The actual case presentation the PowerPoint was developed from, minimum 1000 words with a reference list.
