Case Study Scenario
Casey is an Wakka Wakka First Nations woman who has recently arrived in your area from her hometown of Cherbourg, and moved to the city with her partner Corey for work. At this appointment there is no record of Casey attending antenatal care since the morphology scan. You find the Morphology report on the computer indicating Casey attended the scan 16 weeks ago. At the time the fetus demonstrated normal growth and was assessed to be 19 weeks and 4 days gestation. Casey decides to attend the MGP you work in with Corey also in attendance, along with Margaret who is Coreys mother. Casey appears uncomfortable in the hospital environment and is reluctant to talk; she does not have a hand-held record and there is no record of her in ieMR. Visually, you assess Casey as being in her mid-twenties, with a flat affect and disengaged manner. However, she consents to you performing a physical examination, and agrees to provide urine and blood samples for screening tests.
On examination, you calculate Caseys BMI to be 34 kg/m2, her blood pressure is 125/77. Temperature is normal, heart rate 110 bpm, respirations 20rpm and breath sounds determined to be normal but you notice a distinct heart murmur. On abdominal examination, you note the fundal height is 32cm and fetus is determined to be in a breech presentation. Fetal movements are palpated, fetal heart rate on doppler varies between 135 -160bpm over one full minute.
Urinalysis and blood screening results:
Protein Nil Haemoglobin 108
Leukocytes ++ Ferritin 6
Glucose + RPR Positive, titre 1:2
Blood + TORCH HSV2 positive
