Case Study

Mrs. Melissa Mendez is a 72- year-old woman who has been in the medical ward for three days after a flare-up of her rheumatoid arthritis, leading to pain and swelling, causing decreased mobility, and restricting her ability to attend to her activities of daily living (ADLs).

She is prescribed the following:

Methotrexate 20mg per oral; weekly

Prednisone 10mg per oral; daily

Simvastatin 40 mg per oral; nocte

On the morning medication round, the RN calls the medical officer as Mrs. Mendez is feeling unwell with symptoms of urinary frequency, dysuria, and urge incontinence. The medical officer diagnoses a urinary tract infection (UTI) and prescribes Trimethoprim 200mg twice daily for three days.

The following morning, during the medication round, the RN is working with a student nurse; they notice Mrs. Mendez has deteriorated and is nauseous and confused. The RN calls a medical officer to review Mrs. Mendez. While waiting for the medical review, the student nurse reviews the medications using the MIMS and identifies drug interactions between Trimethoprim and Methotrexate. The medical officer reviews Mrs. Mendez and changed the Trimethoprim to Co-Amoxiclav because her UTI symptoms have not improved.

After a few days, Mrs. Mendez is feeling much better, her UTI symptoms resolve, and she is discharged from the hospital.
