Instructions:
You are required to complete the attached template as a record of your ability to develop, monitor and review person-centred approaches for clients.
Your activities in this regard must include:

You will need to fill out the template below as you undertake your work it should reflect at least 120 hours of direct support work.
You can refer to different clients (for example, you may have been in involved in the monitoring and review processes for a different client than the one whom you helped develop strategies for, or you may have assisted in developing strategies for more than one client) if so, identify them as Client 1, Client 2.
You will need to add extra rows to your table you can either complete the table using Microsoft Word (your assessor can give you a Word file with the table in it) or you can simply make copies of the table as you need extra space.
Below is guide as to what you need to include in each section of the template.
Remember to consider confidentiality and privacy do not include identifiable information about your client (for example, their name, names of their family members, etc.)

ASSESSMENT TASK 5: WORKPLACE OBSERVATIONS
Task Summary:
There are two parts to this task:
Part A requires you to work with an individual and their family and/or carer to develop a person-centred approach.
Part B requires you to monitor and review the approach (this part may involve a different client to that chosen in Part A).

When do I do this task?
You will do this task during your assessors workplace visit. There are two options to this:
Option 1: Two workplace visits, where you will do Parts A and B with the same client or
Option 2: One workplace visit where you will do Parts A and B with different clients.
Your assessor will discuss this with you and advise you of the most suitable option.

If your assessor sees that you have not shown appropriate skills or knowledge, they will give you some feedback and you will need to do the specific task again.

Note: You will need to choose two different clients if your assessor will be observing this task at the one workplace visit.
You will need to discuss the arrangements of this task with your assessor and supervisor to ensure that you can do this task during your assessors visit/s.
If the clients family is involved, you will need to ensure that the time of your assessors visit is suitable for them.
Part A: Develop a person-centred response
Your assessor will observe you as you work with a client (or their family and/or carer) to develop a person-centered approach.
You will be required to do the following:
Work effectively with the client and all members of their support team to develop an approach
Develop an approach suitable for the clients needs and preferences
Correctly complete all required workplace documentation
Organise any resources required
Seek help or advice from others in the workplace service as required
Following this observation you need to answer a set of questions that your assessor will ask you verbally
Your assessor will be looking to see that you:
Collaborate with the client (or their family and/or carer if applicable) to develop and implement a person-centred approach
For example: this will include communicating with each person in a professional manner; considering the information and requirements received from each member to implement into the approach and so on.
Communicate effectively with the individual, family and/or carer
For example: this will include communicating the needs of the client, asking appropriate questions; using appropriate listening skills; adjusting communication styles as required (for example, visual communication versus written/verbal); using appropriate level of language and grammar; showing empathy; clearly stating information; requesting confirmation that the client/family understands, etc.
Ensure the approach meets the individuals needs, rights, goals and preferences
For example: confirming with the client and members of the support team that the approach and goals are realistic, achievable and aligned with the clients needs, strengths and preferences; modifying aspects of the approach based on feedback, etc.
Accurately record client details, plans and future activities
For example: clearly completing all required documentation; taking notes about what is discussed; maintaining or updating client files or records (manually or electronically as per procedures), etc.
Organise required resources
For example: this might include contacting specific services to help put in place in the approach/plan (for example, organising client services, transport, additional staff) etc.
Identify the need to seek additional assistance from other workers where required
For example: being able to identify and refer instances where the client may require additional services outside of the students own scope, skills or qualification.
Part B: Monitor and review person-centred approaches and responses
For this part of the task your assessor will observe you reviewing and monitoring a person-centered approach and response. (This client may be different to the one you worked with in Part A.)
You will be required to:
Discuss findings and/or observations of the clients responses to the approach
Review with the client (or their family) if the approach was effective
Identify any areas that did not meet the clients needs or did not work
Plan future actions and steps for improvement
Review the approach in line with the services delivery
Follow all relevant workplace procedures.
Following this observation you need to answer a set of questions that your assessor will ask you verbally.
Your assessor will be looking to see that you:
Consult with the client (or their family and/or carer) when reviewing of the approach has been effective
For example: communicating with the client and family to review the clients needs and progress. The student must be able to demonstrate different communication mediums used in the consultation process (this might include phone calls, emails, face to face meetings/discussions).
Review the clients responses to the approach
For example: identifying the clients response to the approach and activities; re-evaluating the plan based on the success/ failure of the clients response or by referring to observations or feedback from the care team.
Identify and take action to improve the approach
For example: re-establishing actions/next steps in modifying the client approach, plan or strategy; defining new activities or modifying existing activities to better suit the clients needs and preferences.
Follow all relevant procedures in line with the services quality system and standards
For example: completing tasks and activities in line with workplace processes and procedures; ensuring high quality service is maintained at all times; working in a professional manner; referring to workplace documentation to ensure they are adhering to standards and protocols, etc.)

Assessment Task About this task
Assessment Task 1: Written questions Students must correctly answer all 17 questions to show that they understand the knowledge required of this unit.
Assessment Task 2: Case studies Students are to read four case studies and complete the questions for each.
Assessment Task 3: Project There are two parts to this assessment.
Part A: Students must develop an equal employment policy and answer a set of written questions.

Assessment Task 4: Project Students must complete multiple parts to demonstrate their ability to monitor compliance.
Part A: Students will be given a scenario and they must identify the breach of compliance. They must then identify and report on the corrective action to be taken.

Case Study 1: Prasad moves to Australia
Prasad recently joined SupportKids, taking on the role of the team leader.
Prasad has recently moved to Australia from India. He was very pleased that his experience with children with disabilities in India has been recognised and he could get employment in a team leaders role.
Prasad had been employed in India for over 15 years, during which time he had campaigned for the rights of children and people with disabilities. Prasad was known to be a skilled and dedicated worker.
After settling into his new role, Prasad noticed some of his team members making quiet jokes when he was around the office. Some would giggle or quietly laugh when he would speak and do silent movements ridiculing his natural gestures.
During lunch times, Prasad overhead some of the team mention his name and put on an Indian accent during their conversation. This happened more obviously when Prasad was giving the team updates during their weekly catch ups.
Furthermore, one of the team members, Ben, would send regular emails to the rest of the team with jokes and comical images about the Indian culture. When Prasad attempted to ignore these jokes about him, he also noticed the responsiveness of his team decline. They were not supportive of his requests and lacked the team cooperation and loyalty that he needed to get tasks done.
Prasad began to lose his confidence with his new role and team leadership. He felt humiliated because of his culture and ethnic background, and he could not understand why his team were not as welcoming as he expected. They did not make him comfortable in his position as their team leader.
Eventually Prasad withdrew his interaction with the team over time, cancelling meetings and confining his communication to emails rather than face-to-face discussion.
The jokes about Prasad failed to cease and became targeted at his withdrawn behaviour. He became agitated and depressed until he completely withdrew himself from the workplace and ceased working due to increasing stress.
Prasad was also homesick, as all of his family and friends resided back in India.

After a few months, the house supervisor advises Kelly that he will be placing her on night shift with the other night worker Thomas.
Kelly is a bit uncomfortable about working with Thomas, especially during the night, but she appreciates the responsibility of working nights. The shift penalties also mean more money in her pay.
When Kelly starts working with Thomas, he casually questions her about her personal life. He asks her questions about whether she has a boyfriend and about her first time sexual encounter.
Kelly finds this intrusive as she has never been asked these type of questions by anyone else in a workplace. But she thought this might just be Thomas trying to be friendly and make an effort to get to know her better.
Kelly has a strong commitment to her job and a loyalty to do her best for her clients.
Thomas begins to give Kelly compliments on how she dresses and presents herself, most especially when she wears a skirt. He makes comments like, If I wasnt already married and I was your age I would be all over you by now!
Kellys discomfort around Thomas grows when he starts to read sexually explicit content in magazines that they confiscate from their clients. He also shows her pornography on the Internet, saying that he has caught the clients accessing it.

Case Study 3: Helping out a friend
Thomas is suspended from work and a disciplinary investigation process is in progress.
Thomas calls, Ben, who works in the organisations HR department.
Ben is a friend of his from years ago. They used to play football together and often go out for drinks on Thomass nights off. They arrange to meet at the pub that night.
Thomas buys Ben a beer and fills him in on what has happened he has been wrongfully suspended from work! According to Thomas, Kelly has thrown herself at him during night shift and when he threatened to report her, she got in first and said it was him who assaulted her!
Ben had heard something about the suspension and was very glad to hear that his friend was not in the wrong.
Thomas asks Ben if he could get the disciplinary file out of the HR Managers office and photocopy it for him so he knows what is happening in the investigation. This will help him as he will be able to see Kellys statement and what the investigator has said in the report.
He needs the information before he goes in front of the investigation committee next week.
If Ben can get the report, Thomas promises that he will never tell anyone.
Ben feels sorry for his friend and the next day he works back late. He finds the HR Managers office key and borrows the file. He makes a copy.
On his way home he drops in at the pub and gives Thomas the photocopies. Ben cant believe the lies that Kelly has told about Thomas in her statement he feels really sorry for his friend as it looks like it will just be Kellys word against his, and everyone knows that the female is always believed over the male in a sexual assault case!

He had to be taken to hospital he had fractured his wrist.
After all the drama of Martins accident died down, Jenny decided to put all the stationery away herself.
She had almost got everything done, but the last items were six boxes of photocopy paper.Jenny used the ladder as instructed by the procedures to put the boxes on the top shelf.By the time she got to the fourth box she was exhausted it was hard work lifting boxes up a ladder!
Just was she was about to put the box on the shelf, her foot slipped on the ladder step. She fell onto the floor thankfully the box did not hit her! However, Jenny still ended up with cuts on her leg from the metal ladder, and lots of bruises and a very sore ankle and arm.
Naomi is the new manager of the organisation. She has been on the job for not even two weeks. She is horrified that two accidents could occur on the same day, and in the same place.
Naomi investigates the incident and when she reads the procedure she is shocked at the manual handling processes that staff are being advised to carry out.She is surprised there hasnt been more serious accidents.

ASSESSMENT TASK 2: ROLE PLAY
Task summary:
There are two parts to this task:
Part A: You are to respond to a critical incident involving a behavioural issue of a person with a disability.
Part B: You are required to answer a set of questions about effective response strategies.
Part A Role play

You will do this task during your assessors workplace visit.

If your assessor identifies that you have not demonstrated the required skills/knowledge, they will give you some feedback and you will need to do the role play again.
Instructions:
The purpose of this task is for your assessor to observe how well you can respond to a critical incident involving adverse behaviour in a simulated environment, using appropriate support strategies and communication. You will do this role play in your workplace with your assessor.
First you will need to watch the YouTube video titled Autism, Self-Injurious Behavioral Interventions. You should do this prior to your assessors workplace visit.

The video shows the self-injurious behaviour of James, who has severe autism and epilepsy. James is seen repeatedly hitting his head in a violent manner with both hands.
You will then need to participate in a role play with your assessor playing the role of James. James has just been disturbed by a car going past and squealing its brakes.
You will be required to do the following during the role play:
Use appropriate interventions and positive behavioral support strategies.
Use a person-centred approach.
Address safety requirements and procedures.
Use effective communication strategies to meet Jamess needs.
Jamess behavioural management plan states the following:
James is often startled by loud noises, which cause a buzzing in his head.
He becomes self-injurious, hitting himself around the head involuntarily. He may also experience a seizure.
James only ever hits himself never anyone or anything else.
James responds to wearing headphones to reduce noise stimulus. These should be put on as soon as possible during an incident.
James may wear padded mitts to prevent injury.
James will respond to calm, repetitive instructions.
Holding Jamess hands in his lap reduces the risk of injury and can be calming. James does not find this restrictive.
James reacts badly to sudden loud noises, shouting, complicated instructions, laughing or panic by anyone near him.
James behaviour can be diverted by drawing his attention to objects he enjoys for example, watching the ceiling fan, his pet dog, his toys and other family members.
Your assessor will be looking to see that you:
Identify the triggers that cause Jamess adverse behaviour.
Respond to James in an appropriate manner, correctly carrying out behavioural strategies in response to his behaviour.
For example remaining calm, being consistent, showing empathy, following the details in his behavioural management plan, etc
Consider and acknowledge Jamess needs, strengths, capabilities and his preferences.
For example, acknowledge attempts made by James to control his behaviour; his likes and dislikes; keeping instructions simple; need for particular intervention strategies, etc.
Make sure Jamess environment is safe.
For example, by protecting Jamess head and hands; by keeping your own self safe; reducing possibility of his triggers occurring again, etc
Acknowledge Jamess emotional state.
For example, by demonstrating empathy, acknowledging his distress, acknowledging him as he calms down, etc
