For this part of the assessment task, you will be required to identify and gather the forms the service uses for each of the policies identified in Task 1.1. You must complete an example form for each policy and submit this for assessment. The forms should be completed in a way that would demonstrate to a new inducting staff member the expectations for filling in the form.
You must ensure that you use one form form per policy and then compile them in one single document. Ensure that you follow the instructions provided in the steps below.
1. Collect forms associated with service policies
2. Complete forms
3. Identify and report a hazard
Step 1: Collect forms associated with service policies
Collect all forms associated with the following policies:
Health, safety & wellbeing policies/procedures, including supervision and infection control requirements
Incidents, injury, trauma and illnesses policies/procedures
Medical conditions policies/procedures
Administration of medication policies/procedures

Report this hazard to your workplace supervisor and the appropriate person based on the policy (i.e. the WHS officer or Centre Director).
Document who you reported the hazard to (this might be included on the service form).
Upload a copy for assessment labelled CHCECE041_WOA_Task_1-3.

Part 2: Design, risk assess, and participate in an excursion
-259080-25590500Part overview
For this task, you are required to design a walking excursion.
You must scope out the location, facilities and activities prior to the excursion to identify hazards.
You must prepare the risk assessment on the Excursion Risk Management Plan template (below), which includes assessing risk, preparing for the excursion, planning and facilitating group learning discussion with children, and critically reflecting on the excursion.
If your workplace chooses not to conduct the excursion, you may conduct a pretend walking excursion in the yard with a small group of children and your workplace supervisor. The Excursion Risk Management Plan template must reflect the real excursion that you have planned.
Your supervisor will need to confirm you have completed some tasks by signing the observation checklist.
Task 2 Instructions
The instructions for each part of the Excursion Risk Management Plan are as below:
You must access the location, facilities and activities prior to the excursion to identify hazards. Document the notes you take and safety considerations in the Excursion Risk Management Plan template.
You must write the details of the excursion in a parent communication format to ensure they are aware of what to expect for their childs involvement in the excursion.
You must complete the excursion checklist on the Excursion Risk Management Plan template by collecting and/or checking all items on the list. If you complete the excursion, all items required to be taken on the excursion must be present for the excursion.
You must complete the risk assessment using the risk matrix on the Excursion Risk Management Plan template. You must complete a risk assessment for at least seven identified hazards and may add additional lines where necessary. Where hazards are identified that pose too great a risk, the excursion must be cancelled.
Prior to the excursion, you must plan and prepare a group learning discussion with children prepare them for the excursion. Document your plan for the discussion and your intentional teaching strategies on the Excursion Risk Management Plan template.
Prior to the excursion, you must facilitate the discussion with children and use the intentional teaching techniques to prepare them for the excursion. Your supervisor must observe this group learning discussion.
