Read through the following case study on Mr Wilson below:

Mr Wilson is a 76-year-old gentleman born on 05.02.1944. Mr Wilson has been admitted to Federation University Hospital onto the acute medical ward with diarrhoea and vomiting for investigation.

Mr Wilson has a history of hypertension, GORD and osteoarthritis.

Clinically, Mr Wilson is experiencing dizziness and weakness. He is currently resting in bed as he feels too fatigued and dizzy to walk around. He has not been able totolerate oral intake and is just having sips of water.

You are the nurse caring for Mr Wilson this morning. During your bed bath, you notice he has an area of non-blanchable redness over his sacrum. When you apply gentle pressure, Mr Wilson states he feels some pain.

After attending to Mr Wilson's hygiene needs, you encourage him to get out of bed and sit in his hair. He states he does not want to as he is afraid of falling due to the dizziness.

Using the following headings as a guide, outline the nursing care required for the case:

  1. Consider the patient situation (1 min) - Consider Mr Wilson's situation and identify any relevant facts. Write your initial impression of Mr Wilson based on his current situation including the context, people and situation.
  2. Collect cues/information (1 min) - After reading through Mr Wilson's story, what relevant cues and information can you identify? For example, what current information is of interest, what other information or assessments might provide useful information? What signs and symptoms do you notice about Mr Wilson's clinical presentation?
  3. Process information (1 min) - From the cues and information you collected in the previous phase, what facts and inferences can you make? Are they any abnormal findings, cues to cluster together or patterns that you can identify?
  4. Identify problems/issues (1 min) - Synthesise the facts and inferences of the information collected in the previous phase and consider the most pertinent potential and actual problems for Mr Wilson. From this list, chose the most pertinent health issue to discuss for Mr Wilson and provide reasoning for your choice.
  5. Establish goals (1 mins) - Thinking about the health issue you have identified for Mr Wilson in the previous phase, identify one goal of care for him.
  6. Take action (2 mins) - Thinking about the goal you establish for Mr Wilson in your previous phase, how can you facilitate this as a nurse? What is the most appropriate nursing intervention?
  7. Evaluate outcomes (2 mins) - If you were able to implement the above intervention, how would you evaluate the effectiveness of this care to ascertain if the action has improved his condition?
  8. Reflect on the process and new learning (1 min) - Reflecting on this process, identify your strengths and weaknesses of working through the clinical reasoning cycle. Identify one weakness and discuss a plan for improvement of this element.
