Mr W is a 52-year-old Caucasian male, unemployed and separated from his wife. He has two children who live interstate. He currently lives in a two-bedroom housing commission flat on the second floor. He has been trying to lose weight and increase his exercise for the past 6 months without success. He drinks two bottles of beer every night and a glass of wine with dinner. He also smokes 8 cigarettes a day.
Relevant Medical History:
Diabetes mellitus type 1, kidney failure on haemodialysis X 3 times a week
Chronic back pain which he sustained 10 years ago on a building site
Depression
Substance dependence
Coronary artery disease (CAD), was diagnosed 3 months ago
Obesity
Presenting Problem:
Mr W has revealed that he has given up hope and has stopped taking his antidepressant medications. Mr W had indicated he is not in a good relationship with his two sons and that he has not seen either of them for the past 3 years. Mr W has also indicated that he is lonely since he does not go out due to his mobility issues. His GP has recently referred him to a vascular surgeon for a review of a black discolouration on his right big toe.
