Information on the placement agency:I am completing my placement at Eremea, an elderly in-home care provider, where I have been assigned a caseload of high-risk, vulnerable clients who have been awarded government fundingto help them live as independently and comfortably at home as possible. Eremea serves a wide range of clients from various cultural backgrounds, each of whom has individual needs.
Case management contains four core elements: assessments, case planning, implementation (service delivery) and monitoring and reviewing. Case management allows for the elderly to be advocated for, arranges in-home and community support, and ensures they can maintain a level of independence whilst not having to worry about accessing resources. Additionally, case management allows for adjustment to services to meet the changing needs of the client, the client's family, and the ongoing changes to the Aged Care system. Navigating the aged care system and all the providers has proven to be challenging and time-consuming, having case management allows for the client and the family to access services without having to add on additional stress and allows more time to focus on each other and living their best lives.
