Section 1: The MSE
The MSE should be prepared as though you were transmitting information about Alison to another healthcare professional. The MSE should be written with conventional headings. The headings are Appearance, Behavior, Mood and Affect, Speech, Thought Form, Thought Content, Perception, Cognition, Insight and Judgement, Risk. Make that in table format
Documented observations must meet four criteria to be awarded marks.
Observations must be:

appropriately categorized,
clear and
aptly detailed
The observation documented must be consistent with the video.
Section 2: The Report
Based on the information from the MSE and the scenario below, you are to write a 1000-word report about co-developing a care plan with Alison that reflects her recovery goals.
Scenario:Two weeks after the interview, Alison presented for a medical review and informed Dr Taylor that her mood had not improved; she had poor sleep and appetite and was finding it increasingly difficult to care for her two children. She also told Dr Taylor that she was sick of her job in the supermarket and dreams of enrolling in an accounting degree, as she wants a better life for herself and her children. Alison also said she wants to get better so she can be more present for her children, travel overseas, socialise with friends and recommence drawing and painting, a hobby she enjoys. Alison is open to Dr Taylors suggestion of a support group, so she can receive support from others who experience depression. Dr Taylor admits Alison to the mental health unit you are working in, to trial a new antidepressant. You have been assigned to care for Alison and decided to chat with her about her recovery goals.
