Purpose
Case studies are analyses of situations or problems to be solved. They are used to communicate challenges and recommendations, in order to drive change. In this assessment, you will apply theories taught in class to a real world scenario, helping you develop your critical thinking skills.
Task Details
This assessment task requires you apply the clinical reasoning cycle to the Case Study provided in the Resources section. Use the Assessment Template provided to prepare a plan of care (based on the clinical reasoning cycle) for the older person in the Case Study
As part of this assessment, you are required to:
1. Describe provide an overview of the scenario (consider the person and context. Location people situation) (approx. 50 - 100 words)
2. Collect Cues Identify 6 relevant cues related to current information and 6 potential assessments that you would use to gather new information. Include a rationale for the additional assessment tools (approx. 150 - 200 words)
3. Process Information Identify 3 priority cues from the assessment findings (ie. information contained in the case study) and explain why the cues are a priority for the older person. Reference/s needed (approx. 200 words)
4. Synthesise Identify one actual nursing diagnosis older person and one potential nursing diagnosis for the older person (100 words)
5. Establish goals - Identify one person-centred goal for the older person that addresses any of the identified nursing diagnosis (50 words)
6. Select a course of action outline two (2) nursing interventions for the identified goal that will assist the older person to meet the desired outcomes. Provide brief evidence-based rationales for each intervention. Reference/s needed. (approx. 300 words)
7. Evaluate outcomes Identify the process for evaluating the desired outcomes in this scenario (approx. 100 words)
8. Reflect Use the SPROUT model of reflection to consider on what you have learned from completing this task and how it will contribute to your future practice (approx. 250 words)
