Task description
For this assessment task, you are required to formulate a case for a person with a disability for whom you are currently providing behaviour support services, or have in the past, who is at risk or in contact with the justice system.
Please ensure there are no current legal impediments, such as a current Suppression Order, preventing you from referring to the experiences of the person you have chosen in your assessment task.
IMPORTANT
To protect the privacy of the individual on whom you are basing your case formulation, please ensure they cannot be identified in the details you provide. You should also de-identify the individual in any discussions you have about aspects of your assessment task with peers and with your Course Coordinator.

1 For the person with a disability you have chosen for this assignment, identify the relevant assessments and reports that are considered key information sources required to understand them and their behavioural presentation.
NOTE: These information sources will include structured clinical judgement risk assessments such as the ARMIDILO, functional behaviour assessments, key allied health assessments, file reviews and potentially a clinical interview with the person.
2 Using the information derived from Step 1, develop aconcise case formulation (abstract length of 500 words)about the person, that incorporates their relevant risk factors, protective factors, and key needs using the 5Ps framework as illustrated below:

Task description
This assessment task requires you to create aSafety Planfor the person you based your case formulation on in Assessment 1.
The safety plan functions as a supporting template to a comprehensive or interim Behaviour Support Plan under the NDIS, by translating findings from the case formulation into a practical support plan aimed at managing the persons behaviours of concern and minimising the communitys risk of harm, in the short and long term.
The Safety Plan should ideally:
Follow a proactive risk management approach that aligns with the Positive Behaviour Support Framework
Be developed in consultation with the person and their care team.
If you were provided with an alternative person in Assessment Task 1 by the Course Coordinator because of legal impediments to using an actual person, you will be advised about an alternative to consultation with the person and their complex care team.

To assist you in creating a Safety Plan, we have provided the followingtemplate, together with step-by-step instructions below.
Steps Descriptions
1 UnderSection 1: Risk Management Planof the safety plan template, identify the most relevant areas of need for the person to improve their quality of life, minimise harm, and avoid further contact with the criminal justice system.
This step should include:
Identifying the persons problem behaviour, personal goals in relation to quality-of-life domains, and any legal orders and restrictions that need to be considered in planning.
Identifying the assessed high risks that require immediate management and what protective factors are deemed highest and most relevant to the situation that need to be maintained, or strengthened, as part of the risk mitigation process.
Presenting risk factors and protective factors as separate individual factors, as well as the environmental factors related to the person.
Identifying situations that may require increased support or supervision as part of the short-term risk management process of the safety plan.
2 UnderSection 2: Needs Based Intervention Planof the template, identify a replacement behaviour goal for the problem behaviour listed under Section 1.
This step should include:
Providing recommendations for shaping behaviour to manage the persons risk of harm towards others, and to maximise their quality of life in the long term.
Making recommendations across different areas of positive programming associated with PBS, including environmental enhancements or changes, personal supports required, and targeted skill development.
3 To complete Sections 1 and 2 in the template, you will need to demonstrate engagement with the persons care team and/or support network and provide a list of persons consulted in Section 3 of the safety plan template.
Remember to only record the role of the people consulted and not their names to protect their identities.
4 In line with the foundations of PBS, you should make every effort to engage the person with a disability who the safety plan is supporting in the process of designing their safety plan.
This should be evident in your identification of personal goals, and the use of language and strategies which would be understood by the person.
