HIRAID is an evidence-informed nursing assessment framework which can be used to evaluate the communication, task management, decision making and situational awareness of nurses related to patient. The initial phase of the assessment is triage phase in which the medical history of the patient is checked and the nature of problem is identified. This is the most crucial phase of healthcare assessment as the information obtained from this stage is carried on to next stages.Then the assessment is performed for checking the signs and symptoms and recognizing the underlying disease. Further in focused assessment phase is done by the emergency nurse. In this phase comprehensive assessment and further investigations are done to provide the interventions and emergency care to meet the patients clinical needs. Emergency nurses must perform accurate and comprehensive patient assessment to determine the right treatment needs of patients (Munroe, B et al, 2015). Based on the interventions and diagnosis a long term treatment plan is developed. After treatment has started a reevaluation is done to check the progress of patient. Then the interaction with the patient is done and the reports are documented.

Cultural and spiritual life stage factors -

The cultural and spiritual life stage factors need to be considered for treatment as they play a significant role for patients recovery. Patients need to be addressed in the language they understand. The reports and treatment process should be translated to them. Apart from that the patients circumstances and current situations need to be addressed.

The patient in the video is suffering from personal issues which is causing a lot of stress to the patient resulting in impact on her health.

The nurses should consider the following points while helping her for the recovery-



  1. Situation awareness - They should know about the patient situation and her background, anticipate her situation and be understanding to her.
  2. Communication - They should talk to her more politely and listen patiently.
  3. Task management - Nurses should actively plan, prepare, identify and utilize the resources adequately for the patient.
  4. Decision Making - In case of emergency nurses should identify the symptoms, choose the best options available and handle the risk situation.
  5. Re- evaluating - After treatment, reevaluation of the patient should be done to check the progress.



Effective Communication Techniques

Effective communication helps in building relationshipbetween the nurse and patient whilemaking patient aware about the therapeutic process and the treatment's results.

Effective communication can benefit the patient in a number of ways in reducing suffering, anxiety and disease symptoms as well as in enhancing patient's acceptance, compliance, and cooperation with the medical staff (Foronda, C et al, 2016). In the first part of the video ineffective communication has been portrayed by the nurse. The nurse was in a hurry and not making eye contact with the patient while talking. She was not talking compassionately, the patient was in stressful situation she should have consoled her and made her comfortable.

The Therapeutic communication technique should be used for an effective communication with the patient. The following points need to be considered-



  1. Active Communication - Active communication requires listening to the patient and asking the essential questions. Nurses should be engaged in the communication with the patient and understand the situation of the patient. For the patient in video nurse should encourage the patient to speak further about the problems she has been facing by showing active participation in the conversation.
  2. Accepting - It's to merely make eye contact while talking, listening to the patient, nodding in response and showing humble gestures the acceptance behaviour can be conveyed. Patients build better relations and trust with the nurses when they feel the sense of acceptance from them (Kourkouta et al, 2014). They start to feel more comfortable and talk about their problems openly.
  3. Giving time to the patient- Spending a few extra seconds with patients can provide them the opportunity to express additional ideas and queries. Patients can disclose potentially crucial details when you make yourself available and willing to spend time with them (Cristea et al, 2015). Additionally, it conveys your concern for the patients.
  4. Observance - Paying attention to the patients appearance and details is must to check for the symptoms. If the patient seems tired, questions like have been sleeping well can be put up. In this way new symptoms can be identified.
  5. Summarizing - Reiterating or summarizing what a patient wordsto elaborate or clarify their symptoms is an approach that is used to avoid any cognitive error. Patients also feel understood as a result.



How Clinical reasoning and shared decision making mitigates risk of cognitive biases portrayed in the video

Cognitive error is pervasive in clinical practice which causes cognitive biases for determining the prevention and treatment care plan for the patient. For the scenario given in the video, mitigation of cognitive biases can start with creating awareness among the workforce. The risk of cognitive errors can be avoided by the healthcare workers by documenting the symptoms of patient with proper name and label. Clinical reasoning should be used by the clinicians and the management to avoid the risk of bias. They should be aware about the case and medical situation of the patient and decide the treatment plan accordingly based on diagnosis and the requirement of the patient (Wiers et al, 2013). Clinicians should analyse weather the treatment and medication will suit the patient or not.

Using shared reasoning approach is the best possible prevention of cognitive biases is the use ofclinical decision support systems. Decisions regarding the treatment plan should be made by allied health professionals for the right service delivery according to the need of the patient. This helps in mitigating the negative impact in clinical practice and quick recovery of the patient.
